This patch (as1519) fixes a bug in the block layer's disk-events
polling. The polling is done by a work routine queued on the
system_nrt_wq workqueue. Since that workqueue isn't freezable, the
polling continues even in the middle of a system sleep transition.
Obviously, polling a suspended drive for media changes and such isn't
a good thing to do; in the case of USB mass-storage devices it can
lead to real problems requiring device resets and even re-enumeration.
The patch fixes things by creating a new system-wide, non-reentrant,
freezable workqueue and using it for disk-events polling.

The following situation might occur:
__blkdev_get: add_disk:
register_disk()
get_gendisk()
disk_block_events()
disk->ev == NULL
disk_add_events()
__disk_unblock_events()
disk->ev != NULL
--ev->block
Then we unblock events, when they are suppose to be blocked. This can
trigger events related block/genhd.c warnings, but also can crash in
sd_check_events() or other places.
I'm able to reproduce crashes with the following scripts (with
connected usb dongle as sdb disk).

I use the script to verify patch fixing oops in sd_revalidate_disk

a few seconds. With both patches applied I do not observe crash.

Anyway I think this patch is some step forward.
As drawback, I do not teardown on sysfs file create error, because I do
not know how to nullify disk->ev (since it can be used). However add_disk
error handling practically does not exist too, and things will work
without this sysfs file, except events will not be exported to user
space.

Since 2.6.39 (1196f8b), when a driver returns -ENOMEDIUM for open(),
__blkdev_get() calls rescan_partitions() to remove
in-kernel partition structures and raise KOBJ_CHANGE uevent.
However it ends up calling driver's revalidate_disk without open
and could cause oops.
In the case of SCSI:

just include the conversion from msecs in the define itself. This has the
advantage of making the code where the timeout is used more readable.

"num_vfs" is a u32 but we only use the high 16 bits and the low 16bits
are left as zero. That isn't a problem for little endian systems but it
will break on big endian ones.

pch_gbe_validate_option() modifies 32 bits of memory but we pass
&hw->phy.autoneg_advertised which only has 16 bits and &hw->mac.fc
which only has 8 bits.

By moving the command handlers into a table (the index being equal to
the opcode) the lookup is made a bit more efficient. Having a struct to
describe each handler also paves the way to add more meta-data for each
handler, e.g. the minimum message size for the command and allow
handling of common tasks like this in a centralized place.

The read_controller_info is typically the first command that user space
sends when taking a controller into use. This is also the reason why
this command has been used as the trigger to set the HCI_MGMT flag.
However, when not running the user-space daemon and using command line
tools it is possible that read_controller_info is not the first
controller specific command. This patch moves the HCI_MGMT
initialization to a generic place where it will be set for whatever
happens to be the first mgmt command targetting a specific controller.

Almost all mgmt commands need to lookup a struct hci_dev based on the
index received within the mgmt headers. It makese therefore sense to
look this up in a single place and then just pass the hdev pointer to
each command handler function.

Add comments for ethtool_cmd::phy_address and
ethtool_cmd::mdio_support, and definitions of the flags currently
used in mdio_support.
In the mdio library, assert that its own flags continue to match those
in the ethtool interface.
In the mii library, use the ethtool flag definition and stop
including <linux/mdio.h>.

The ID packing definitions are needed by userland and the register
definitions may also be useful there.
Do not export mdio_phy_id_{is_c45,prtad,devad}() as the use of bool is
problematic and it's not that useful to export only a subset of these.
Do not export MDIO_SUPPORTS_{C22,C45} directly; these flags are only
exposed to userland through struct ethtool_cmd so they should be
defined alongside that with appropriate names.

By clearing the GPU read domains before waiting upon the buffer, we run
the risk of the wait being interrupted and the domains prematurely
cleared. The next time we attempt to wait upon the buffer (after
userspace handles the signal), we believe that the buffer is idle and so
skip the wait.
There are a number of bugs across all generations which show signs of an
overly haste reuse of active buffers.

Some CSR controllers will generate a spontaneous reset during init and
just eat up any pending command without sending a command complete for
it. This patch solves the issue by just resending whatever was the last
sent command. hci_send_cmd is not used since we need to bypass all other
commands in the send queue.

Carlos was getting
WARNING: at drivers/pci/pci.c:118 pci_ioremap_bar+0x24/0x52()
when probing his sound card, and sound did not work. After adding
pci=use_crs to the kernel command line, no more trouble.
Ok, we can add a quirk. dmidecode output reveals that this is an MSI
MS-7253, for which we already have a quirk, but the short-sighted
author tied the quirk to a single BIOS version, making it not kick in
on Carlos's machine with BIOS V1.2. If a later BIOS update makes it
no longer necessary to look at the _CRS info it will still be
harmless, so let's stop trying to guess which versions have and don't
have accurate _CRS tables.

There's really no point in having hcd->irq as a
signed integer when we consider the fact that
IRQ 0 means NO_IRQ. In order to avoid confusion,
make hcd->irq unsigned and fix users who were
passing -1 as the IRQ number to usb_add_hcd.

This merge is rather big. Here's what it contains:
For am5536udc we have just simple coding style fixes. Nothing that has any
potential to cause any issues going forward.
With mv_udc, there's only one single change removing an unneeded NULL check.
at91_udc also only saw a single change this merge window, and that's only
removing a duplicated header.
The Renesas controller has a few more involved changes. Support for SUDMAC was
added, there's now a special handling of IRQ resources for when the IRQ line is
shared between Renesas controller and SUDMAC, we also had a bug fix where
Renesas controller would sleep in atomic context while doing DMA transfers from
a tasklet. There were also a set of minor cleanups.
The FSL UDC also had a scheduling in atomic context bug fix, but that's all.
Thanks to Sebastian, the dummy_hcd now works better than ever with support for
scatterlists and streams. Sebastian also added SuperSpeed descriptors to the
serial gadgets.
The highlight on this merge is the addition of a generic API for mapping and
unmapping usb_requests. This will avoid code duplication on all UDC controllers
and also kills all the defines for DMA_ADDR_INVALID which UDC controllers
sprinkled around. A few of the UDC controllers were already converted to use
this new API.

USB: MUSB changes for 3.4
Here are a set of changes to the MUSB driver. In summary we have
a patch making modules behave better, there's a fix on debugfs'
error path, a small change removing an unnecessary pm_runtime
call on musb_shutdown() and a fix to relesect the endpoint in
Interrupt context.
This last patch is needed because we must drop musb's lock when
calling request->complete() and that could cause problems if another
thread queues a request and ends up changing MUSB_INDEX register.

This patch add an extra check for BR/EDR and LE-Only discovery.
This way, we are able to return error immediately if the discovery
type requested is not supported by the device.

A small fix for the SSI driver and a fix for system shutdown with modern

with a visible kernel log as the systems they're in tend not to shut
down often and when they do it's usually in form factors that don't have

Per cgroup load-balance has numerous problems, chief amongst them that
there is no real sane order in them. So stop pretending it makes sense
and enqueue all tasks on a single list.
This also allows us to more easily fix the fwd progress issue
uncovered by the lock-break stuff. Rotate the list on failure to
migreate and limit the total iterations to nr_running (which with
releasing the lock isn't strictly accurate but close enough).
Also add a filter that skips very light tasks on the first attempt
around the list, this attempts to avoid shooting whole cgroups around
without affecting over balance.

There are two issues on the default delivery system setting for smsusb:
1) instead of filling the delivery system for the per-client
frontend.ops, it were changing the global structure;
2) The client->frontend.ops copy were keeping the previous value
of the template. So, the first time the device was inserted,
it was using the wrong value.

memblock allocator aligns @size to @align to reduce the amount
of fragmentation. Commit:
7bd0b0f0da ("memblock: Reimplement memblock allocation using reverse free area iterator")
Broke it by incorrectly relocating @size aligning to
memblock_find_in_range_node(). As the aligned size is not
propagated back to memblock_alloc_base_nid(), the actually
reserved size isn't aligned.
While this increases memory use for memblock reserved array,
this shouldn't cause any critical failure; however, it seems
that the size aligning was hiding a use-beyond-allocation bug in
sparc64 and losing the aligning causes boot failure.
The underlying problem is currently being debugged but this is a
proper fix in itself, it's already pretty late in -rc cycle for
boot failures and reverting the change for debugging isn't
difficult. Restore the size aligning moving it to
memblock_alloc_base_nid().
