2014-02-21ARM: 7980/1: kernel: improve error message when LPAE config doesn't match CPUThomas Petazzoni
Currently, when the kernel is configured with LPAE support, but the CPU doesn't support it, the error message is fairly cryptic: Error: unrecognized/unsupported processor variant (0x561f5811). This messages is normally shown when there is an issue when comparing the processor ID (CP15 0, c0, c0) with the values/masks described in proc-v7.S. However, the same message is displayed when LPAE support is enabled in the kernel configuration, but not available in the CPU, after looking at ID_MMFR0 (CP15 0, c0, c1, 4). Having the same error message is highly misleading. This commit improves this by showing a different error message when this situation occurs: Error: Kernel with LPAE support, but CPU does not support LPAE. The maximum usable buffer size of the s390 debug feature (when using the sprintf_view) is 11 * sizeof(long) (1 pointer for the format string + 10 arguments). When a larger buffer size is specified the additional memory is unused and wasted per debug entry. So reducing the buffer size to its maximum (or to the actual buffer size used) will make more precious debug feature space usable. For pci_msg, chsc_msg, and cio_crw we use the additional usable dbf space to reduce the number of allocated pages. 2014-02-21mm: mask bits from pmd in pmd_lockptr/pmd_huge_pteMartin Schwidefsky
The pmd pointer passed to pmd_lockptr/pmd_huge_pte can point to any entry in a pmd table. With USE_SPLIT_PMD_PTLOCKS==1 the code uses virt_to_page to get a struct page for the pmd table. The virt_to_page function automatically masks the lower PAGE_SHIFT bits from the address. But if the size of a pmd table is larger than PAGE_SIZE the additional bits are not removed from the pmd address and the wrong page struct is used. This patch enables Collaborative Memory Management (CMM) for kvm on s390. CMM allows the guest to inform the host about page usage (see arch/s390/mm/cmm.c). The host uses this information to avoid swapping in unused pages in the page fault handler. Further, a CPU provided list of unused invalid pages is processed to reclaim swap space of not yet accessed unused pages. In a virtualized environment and given an appropriate interface the guest can mark pages as unused while they are free (for the s390 implementation see git commit 45e576b1c3d00206 "guest page hinting light"). For the host the unused state is a property of the pte. This patch adds the primitive 'pte_unused' and code to the host swap out handler so that pages marked as unused by all mappers are not swapped out but discarded instead, thus saving one IO for swap out and potentially another one for swap in. 2014-02-21s390/mm,tlb: race of lazy TLB flush vs. recreation of TLB entriesMartin Schwidefsky
Git commit 050eef364ad70059 "[S390] fix tlb flushing vs. concurrent /proc accesses" introduced the attach counter to avoid using the mm_users value to decide between IPTE for every PTE and lazy TLB flushing with IDTE. That fixed the problem with mm_users but it introduced another subtle race, fortunately one that is very hard to hit. The background is the requirement of the architecture that a valid PTE may not be changed while it can be used concurrently by another cpu. The decision between IPTE and lazy TLB flushing needs to be done while the PTE is still valid. Now if the virtual cpu is temporarily stopped after the decision to use lazy TLB flushing but before the invalid bit of the PTE has been set, another cpu can attach the mm, find that flush_mm is set, do the IDTE, return to userspace, and recreate a TLB that uses the PTE in question. When the first, stopped cpu continues it will change the PTE while it is attached on another cpu. The first cpu will do another IDTE shortly after the modification of the PTE which makes the race window quite short. To fix this race the CPU that wants to attach the address space of a user space thread needs to wait for the end of the PTE modification. The number of concurrent TLB flushers for an mm is tracked in the upper 16 bits of the attach_count and finish_arch_post_lock_switch is used to wait for the end of the flush operation if required. The finish_arch_post_lock_switch is called at the end of the task switch after all locks have been released. In concept it is paired with the switch_mm function, but the current code only does the call in finish_task_switch. Add the call to idle_task_exit and use_mm. One use case for the additional calls is s390 which will use finish_arch_post_lock_switch to wait for the completion of TLB flush operations. The types 'size_t' and 'unsigned long' have been used randomly for the uaccess functions. This looks rather confusing. So let's change all functions to use unsigned long instead and get rid of size_t in order to have a consistent interface. The only exception is strncpy_from_user() which uses 'long' since it may return a signed value (-EFAULT). There are only two uaccess variants on s390 left: the version that is used if the mvcos instruction is available, and the page table walk variant. So there is no need for expensive indirect function calls. By default the mvcos variant will be called. If the mvcos instruction is not available it will call the page table walk variant. For minimal performance impact the "if (mvcos_is_available)" is implemented with a jump label, which will be a six byte nop on machines with mvcos. After that the device driver does the initialization of its private data. This can race with unsolicited interrupts which can happen once the device is enabled for interrupts. Split ccw_device_probe_console into ccw_device_create_console and ccw_device_enable_console and reorder the initialization of the ccw console drivers. While at it mark these functions as __init. 2014-02-21s390/cio: fix driver callback initialization for ccw consolesSebastian Ott
ccw consoles are in use before they can be properly registered with the driver core. For devices which are in use by a device driver we rely on the ccw_device's pointer to the driver callbacks to be valid. For ccw consoles this pointer is NULL until they are registered later during boot and we dereferenced this pointer. This worked by chance on 64 bit builds (cdev->drv was NULL but the optional callback cdev->drv->path_event was also NULL by coincidence) and was unnoticed until we received reports about boot failures on 31 bit systems. Fix it by initializing the driver pointer for ccw consoles. 2014-02-21s390/cio: Fix missing subchannels after CHPID configure onPeter Oberparleiter
Performing a Channel-Path configure on operation on a Channel-Path ID (CHPID) does not trigger a scan for subchannels that might have become available through that CHPID. As a result, some subchannels and associated I/O devices might be missing. Fix this by adding the missing scan. This problem was introduced by commit c820de39, "[S390] cio: Rework css driver.", but wasn't noticed earlier because the machine usually also generates a Channel-Report-Word when the first CHPID of a subchannel is configured on, resulting in a separate scan for that subchannel. The problem only becomes apparent when this first CHPID is not working properly and additional working CHPIDs are subsequently configured on without any effect on the availability of the affected subchannel. 2014-02-20x86, acpi: Fix bug in associating hot-added CPUs with corresponding NUMA nodeJiang Liu
Current ACPI cpu hotplug driver fails to associate hot-added CPUs with corresponding NUMA node when doing socket online. The code path to associate CPU with NUMA node is as below: acpi_processor_add() ->acpi_processor_get_info() ->acpi_processor_hotadd_init() ->acpi_map_lsapic() ->_acpi_map_lsapic() ->acpi_map_cpu2node() cpu_subsys_online() ->try_online_node() ->node_set_online() When doing socket online, a new NUMA node is introduced in addition to hot-added CPU and memory device. And the new NUMA node is marked as online when onlining hot-added CPUs through sysfs interface /sys/devices/system/cpu/cpuxx/online. On the other hand, acpi_map_cpu2node() will only build the CPU to node map if corresponding NUMA node is already online, so it always fails to associate hot-added CPUs with corresponding NUMA node because the NUMA node is still in offline state. For the fix, we could safely remove the "node_online(node)" check in function acpi_map_cpu2node() because it's only called for hot-added CPUs by acpi_processor_hotadd_init(). LFM (max efficiency ratio) is the max frequency at minimum voltage supported by the processor. Using LFM as the minimum P state increases performmance without affecting power. It never actually worked as the address set is different from any others supported by the driver currently. An additional patch enables correct support for this part but is too large to sensibly apply as a fix to some support that never actually allowed the driver to be successfully probed on this part.

2014-02-20tcm_qla2xxx: Perform configfs depend/undepend for base_tpgNicholas Bellinger
This patch performs configfs_depend_item() during TPG enable for base_tpg (eg: non-NPIV) ports, and configfs_undepend_item() during TPG disable for base_tpg. This is done to ensure that any attempt to configfs rmdir a base_tpg with active NPIV ports will fail with -EBUSY, until all associated NPIV ports have been explicitly shutdown and base_tpg disabled. Note that the actual configfs_[un]depend_item() is done from seperate process context, as these are not intended to be called directly from configfs callbacks. 2014-02-20qla2xxx: Fix qlt_lport_register base_vha callback raceNicholas Bellinger
This patch closes a race between qlt_lport_register() and tcm_qla2xxx callback logic by holding qla_tgt_mutex before making the callback. In order for this to work, the qlt_add_target() and qlt_remove_target() code has been changed to avoid the accessing qla_tgt_mutex + list_[add,del] for NPIV enabled ports. This bug introduced in v3.14-rc1 code with commit 49a47f2. 2014-02-20udf: Fix data corruption on file type conversionJan Kara
UDF has two types of files - files with data stored in inode (ICB in UDF terminology) and files with data stored in external data blocks. We convert file from in-inode format to external format in udf_file_aio_write() when we find out data won't fit into inode any longer. However the following race between two O_APPEND writes can happen: CPU1 CPU2 udf_file_aio_write() udf_file_aio_write() down_write(&iinfo->i_data_sem); checks that i_size + count1 fits within inode => no need to convert up_write(&iinfo->i_data_sem); down_write(&iinfo->i_data_sem); checks that i_size + count2 fits within inode => no need to convert up_write(&iinfo->i_data_sem); generic_file_aio_write() - extends file by count1 bytes generic_file_aio_write() - extends file by count2 bytes Clearly if count1 + count2 doesn't fit into the inode, we overwrite kernel buffers beyond inode, possibly corrupting the filesystem as well. Fix the problem by acquiring i_mutex before checking whether write fits into the inode and using __generic_file_aio_write() afterwards which puts check and write into one critical section. 2014-02-20ath9k: Fix ETSI compliance for AR9462 2.0Sujith Manoharan
The minimum CCA power threshold values have to be adjusted for existing cards to be in compliance with new regulations. Newer cards will make use of the values obtained from EEPROM, support for this was added earlier. To make sure that cards that are already in use and don't have proper values in EEPROM, do not violate regulations, use the initvals instead. 2014-02-20brcmfmac: fix txglomming scatter-gather packet transfersArend van Spriel
The driver concatenates multiple packets in one MMC transfer. For scatter-gather to work the total length need to be multiple of 512 bytes. A pre-allocated buffer was used to add padding to accomplish that. However, the length was not properly set and it was freed after the first transfer causing a crash. 2014-02-20mwifiex: fix cmd and Tx data timeout issue for PCIe cardsAmitkumar Karwar
We are sending sleep confirm done interrupt in the middle of sleep handshake. There is a corner case when Tx done interrupt is received from firmware during sleep handshake due to which host and firmware power states go out of sync causing cmd and Tx data timeout problem. Hence sleep confirm done interrupt is sent at the end of sleep handshake to fix the problem.
