memcg_offline_kmem()
memcg_offline_kmem() may be called from memcg_free_kmem() after a css
init failure. memcg_free_kmem() is a ->css_free callback which is
called without cgroup_mutex and memcg_offline_kmem() ends up using
css_for_each_descendant_pre() without any locking. Fix it by adding rcu
read locking around it.

Per the discussion with Joonsoo Kim [1], we need check the return value
of lookup_page_ext() for all call sites since it might return NULL in
some cases, although it is unlikely, i.e. memory hotplug.
Tested with ltp with "page_owner=0".

Commit 7ff9554bb578 ("printk: convert byte-buffer to variable-length
record buffer") introduced a record based printk buffer. Modify
gdbmacros.txt to parse this new structure so dmesg will work properly.

When remapping pages accounting for 4G or more memory space, the
operation 'count << PAGE_SHIFT' overflows as it is performed on an
integer. Solution: cast before doing the bitshift.

When dealing with inline extents, btrfs_get_extent will incorrectly try
to insert a duplicate extent_map. The dup hits -EEXIST from
add_extent_map, but then we try to merge with the existing one and end
up trying to insert a zero length extent_map.
This actually works most of the time, except when there are extent maps
past the end of the inline extent. rocksdb will trigger this sometimes
because it preallocates an extent and then truncates down.
Josef made a script to trigger with xfs_io:

This was introduced in commit f4a66c204482 ("misc: mic: Update MIC host
daemon with COSM changes") and is fixed by reverting the changes to the
size argument of these snprintf statements.

This is the same issue fixed in commit dcf5341f0150 ("ARM: dts:
sun8i-q8-common: Do not set constraints on dc1sw regulator").
Commit message copied:
dc1sw is an on/off only regulator and as such it cannot have constraints.
This is a limitation of the kernel regulator implementation which resolves
supplies on the first regulator_get(), which is done after applying
constraints, and applying the constrains will fail because it calls
_regulator_get_voltage() and _regulator_do_set_voltage() both of which
will fail on a switch regulator when there is no supply (yet).
This causes registering of all axp22x regulators to fail with the
following errors:

[ 1.405131] axp20x-regulator axp20x-regulator: Failed to register dc1sw
[ 1.412436] axp20x-regulator: probe of axp20x-regulator failed with error -22
This commit removes the constrains on dc1sw / vcc-lcd fixing this problem.
Note that dcdc1 itself is contrained to the exact same values, so this
does not change anything.

Just fallout from switching from asciidoc to sphinx/rst.
v2: Found more. Also s/\//#/ in the vgpu ascii-art - sphinx treats
those as comments and switch to variable-width, which wreaks the
layout.
v3: Undo some of the hacks, rebasing onto latest version of Jani's
series fixed it.

In commit
2c1ea4c700af ("EDAC, sb_edac: Use cpu family/model in driver detection")
we switched from using PCI ids to determine which platform we are
running on to using CPU model instead.
I forgot that Broadwell-DE has its own distinct model number different
from Broadwell-EP or -EX.
Fixing this isn't just adding a line to the array of cpuids - the
exising code assumed a 1:1 mapping between entries in that array and the
"enum type" values. Added the type to pci_id_table structure to remove
this dependency and allows two Broadwell cpu models.

Move the contents of the video/omapdss.h header file to omapdrm/dss local
header file and remove the original global header. The omapfb stach is
using video/omapfb_dss.h so this change will complete the separation of the
two driver implementation.

Copy the content of video/omapdss.h to a new (video/omapfb_dss.h) header
file and convert the omapfb drivers to use this new file.
The new header file is needed to complete the separation of omapdrm and
omapfb implementation of DSS.

The default_device is no longer used, it is a leftower from legacy. The
else if (pdata->default_device) is always going to be false.

The driver only supports composite connection when booted in legacy mode
so the omap_dss_venc_type can be dropped from the pdata.
At the same time the video/omapdss.h include can be removed as it is no
longer needed.

In legacy mode (non DT mode) support only composite connector type. The
only user for this is rx51, using composite type.
Dropping the connector_type selection via pdata will allow cleanups in
omapdss (drm vs fbdev).

Some panel/encoder/connector driver includes omap-panel-data.h but they
do not need it. Remove the inclusion of video/omap-panel-data.h from these
drivers.
