memblock is now fully integrated into the kernel and is the prefered
method for tracking memory. Rather than reinvent the wheel with
meminfo, migrate to using memblock directly instead of meminfo as
an intermediate.

Due to a design incompatibility between the PCIe Marvell controller
and the Cortex-A9, stressing PCIe devices with a lot of traffic
quickly causes a deadlock.
One part of the workaround for this is to have all PCIe regions mapped
as strongly-ordered (MT_UNCACHED) instead of the default
MT_DEVICE. While the arch_ioremap_caller() mechanism allows
sub-architecture code to override ioremap(), used to map PCIe memory
regions, there isn't such a mechanism to override the behavior of
pci_ioremap_io().
This commit adds the arch_pci_ioremap_mem_type variable, initialized
to MT_DEVICE by default, and that sub-architecture code can
override. We have chosen to expose a single variable rather than
offering the possibility of overriding the entire pci_ioremap_io(),
because implementing pci_ioremap_io() requires calling functions
(get_mem_type()) that are private to the arch/arm/mm/ code.

The regression is caused by the size limitation of the x86 early IO mapping.
The root cause is:
1. ACPICA doesn't split IO memory mapping and table mapping;
2. Linux x86 OSL implements acpi_os_map_memory() using a size limited fix-map
mechanism during early boot stage, which is more suitable for only IO
mappings.
This patch fixes this issue by utilizing acpi_gbl_verify_table_checksum to
disable the table mapping during early stage and enabling it again for the
late stage. In this way, the normal code path is not affected. Then after
the code related to the root cause is cleaned up, the early checksum
verification can be easily re-enabled.
A new boot parameter - acpi_force_table_verification is introduced for
the platforms that require the checksum verification to stop loading bad
tables.
This fix also covers the checksum verification for the table overrides. Now
large tables can also be overridden using the initrd override mechanism.

It is reported that Linux x86 kernel cannot map large tables. The following
large SSDT table on such platform fails to pass checksum verification and
cannot be installed:

It sounds strange that in the 64-bit virtual memory address space, we
cannot map a single ACPI table to do checksum verification. The root cause
is:
1. ACPICA doesn't split IO memory mapping and table mapping;
2. Linux x86 OSL implements acpi_os_map_memory() using a size limited fix-map
mechanism during early boot stage, which is more suitable for only IO
mappings.
ACPICA originally only mapped table header for signature validation, and
this header mapping is required by OSL override mechanism. There was no
checksum verification because we could not map the whole table using this
OSL. While the following ACPICA commit enforces checksum verification by
mapping the whole table during Linux boot stage and it finally triggers
this issue on some platforms:

Before doing further cleanups for the OSL table mapping and override
implementation, this patch introduces an option for such OSPMs to
temporarily discard the checksum verification feature. It then can be
re-enabled easily when the ACPICA and the underlying OSL is ready.
This patch also deletes a comment around the limitation of mappings because
it is not correct. The limitation is not how many times we can map in the
early stage, but the OSL mapping facility may not be suitable for mapping
the ACPI tables and thus may complain us the size limitation.
The acpi_tb_verify_table() is renamed to acpi_tb_verify_temp_table() due to the
work around added, it now only applies to the table descriptor that hasn't
been installed and cannot be used in other cases. Lv Zheng.

lock_parent() very much on purpose does nested locking of dentries, and
is careful to maintain the right order (lock parent first). But because
it didn't annotate the nested locking order, lockdep thought it might be
a deadlock on d_lock, and complained.
Add the proper annotation for the inner locking of the child dentry to
make lockdep happy.
Introduced by commit 046b961b45f9 ("shrink_dentry_list(): take parent's
->d_lock earlier").

Make it a little clearer what the littleendian access macros in
vdso2c.[ch] actually do. This way they can probably also be moved to
a central location (e.g. tools/include) for the benefit of other host
tools.
We should avoid implementation namespace symbols when writing code
that is compiling for the compiler host, so avoid names starting with
double underscore or underscore-capital.

The universal/local bit handling was incorrectly done in the code.
So when setting EUI address from BD address we do this:
- If BD address type is PUBLIC, then we clear the universal bit
in EUI address. If the address type is RANDOM, then the universal
bit is set (BT 6lowpan draft chapter 3.2.2)
- After this we invert the universal/local bit according to RFC 2464
When figuring out BD address we do the reverse:
- Take EUI address from stateless IPv6 address, invert the
universal/local bit according to RFC 2464
- If universal bit is 1 in this modified EUI address, then address
type is set to RANDOM, otherwise it is PUBLIC
Note that 6lowpan_iphc.[ch] does the final toggling of U/L bit
before sending or receiving the network packet.

When checking whether we need to request authentication or not we should
include HCI_SECURITY_FIPS to the levels that always need authentication.
This patch fixes check for it in the hci_outgoing_auth_needed()
function.

In case there are new LTK types in the future we shouldn't just blindly
assume that != MGMT_LTK_UNAUTHENTICATED means that the key is
authenticated. This patch adds explicit checks for each allowed key type
in the form of a switch statement and skips any key which has an unknown
value.

We have callers outside of the blk-mq proper (like timeouts) that
want to call __blk_mq_complete_request(), so rename the function
and put the decision code for whether to use ->softirq_done_fn
or blk_mq_endio() into __blk_mq_complete_request().
This also makes the interface more logical again.
blk_mq_complete_request() attempts to atomically mark the request
completed, and calls __blk_mq_complete_request() if successful.
__blk_mq_complete_request() then just ends the request.

Intel Wired LAN Driver Updates
This series contains updates to i40e and i40evf.
Kevin updates the i40e and i40evf driver i40e_check_asq_alive() to ensure
the length register offset is non-zero which indicates that the software
has initialized the admin queue. Also removes PCTYPE definitions which are
now reserved.
Mitch enables descriptor prefetch for rings belonging to the virtual function.
Also configures the VF minimum transmit rate to 50 Mbps rather than 0 which was
be interpreted as no limit at all. Mitch found in order for the VF to achieve
its programmed transmit rate, we need to set the max credit value to 4.
Lastly fixes a Tx hang and firmware crash that happens after setting the MTU
on a VF by not using the RESETTING state during reinit, this is because
the RESETTING state means that a catastrophic hardware bad thing is happening
and the driver needs to tiptoe around and not use the admin queue or registers.
A reinit is no big deal and we can use the admin queue (and we should) so
do not set the state to RESETTING during reinit to resolve the bug.
Akeem changes the declaration of the transmit and receive rings inside
several loops to eliminate declaring the same ring every time for the
duration of the loop and declares them just once before the loop. Also fixes
the driver to clear the recovery pending bit if pf_reset fails instead of
falling through the setup process.
Anjali makes a change based on feedback from Ben Hutchings that cmd->data
needs to be reported in ETHTOOL_GRXCLSRLCNT and use a helper function to
calculate the total filter count.
Jesse removes storm control since the storm control features are not apart
of the hardware and were mistakenly left in the code.
Greg changes tx_lpi_status and rx_lpi_status from bool to u32 to avoid
sparse errors.
Shannon adds the clear_pxe AdminQ API call to tell the firmware that the
driver is taking over from PXE. In addition, relaxes the firmware API
check to allow more flexibility in handling newer NICs and NVMs in the field.
Vasu ensures that FCoE is disabled for MFP modes since it is not supported
by overriding the hardware FCoE capability.

In case of transport HIPER a sock struct is allocated for an incoming
connect request. If the backlog queue is full this socket is not
needed, but is left in the list of af_iucv sockets. Final socket
release posts console message "Attempt to release alive iucv socket".
This patch makes sure the new created socket is cleaned up correctly
if the backlog queue is full.

If a socket is bound to an address using before calling connect
it is usual to leave it to the network system to choose an appropriate
outgoing application name respective port address.
af_iucv on VM uses a counter and uses simple numbers as unique identifiers.
This behaviour was missing when af_iucv is used with HiperSockets.
This patch contains a simple approach to harmonize af_iucv's behaviour.

The MCC async event processing code has 2 issues:
a) because of long struct names the code indentation is badly broken
b) description and definitions of how an MCC completion is interpreted as
an async event are confusing (for e.g. the last word of an MCC event is
named "code", while "code" is just a sub-field of the last word.)
This patch fixes the structure definitions, comments and re-factors code
as needed.

For some FW cmds, the caller just issues the cmd and doesn't wait for a
response. The response handling is done in the MCCQ compl processing context
only. Move this code into a separate routine to make be_mcc_compl_process()
more manageable.

This patch improves MCCQ error status handling in the following ways:
a) A MCC cmd completion returns a base-status and an addl-status.
So far, the routine be_mcc_compl_process() returned only the "status" value.
Now, embedd both statuses in the return value and let the caller routine access
the value of interest using base_status() and addl_status() macros.
b) Rename variables accordingly (base/addl) to avoid confusion while error
checking.
b) Some of the errors returned by FW are harmless and so an error msg is not
logged for such errors. Capture this logic in a separate routine to make the
code more readable.

Certain new flash regions have been added to Skyhawk-R FW image. The newer
FW images specify op_types for each region. A region is flashed only
when it's CRC doesn't match that of the region on the HW flash. While
upgrading to a new FW image the driver is expected to tolerate certain
errors.
This patch re-factors code under be_flash() to support the above scheme.

Skyhawk-R FW supports TX-rate setting only as a % value of the link
speed, set via the SET_PROFILE_CONFIG cmd.
This patch makes the necessary changes to the FW cmd descriptors to support
the above change and also introduces checks in be_set_vf_tx_rate() to allow
only discrete values (that map to % of the link-speed).
