Currently fsnotify framework creates one event structure for each
notification event and links this event into all interested notification
groups. This is done so that we save memory when several notification
groups are interested in the event. However the need for event
structure shared between inotify & fanotify bloats the event structure
so the result is often higher memory consumption.
Another problem is that fsnotify framework keeps path references with
outstanding events so that fanotify can return open file descriptors
with its events. This has the undesirable effect that filesystem cannot
be unmounted while there are outstanding events - a regression for
inotify compared to a situation before it was converted to fsnotify
framework. For fanotify this problem is hard to avoid and users of
fanotify should kind of expect this behavior when they ask for file
descriptors from notified files.
This patch changes fsnotify and its users to create separate event
structure for each group. This allows for much simpler code (~400 lines
removed by this patch) and also smaller event structures. For example
on 64-bit system original struct fsnotify_event consumes 120 bytes, plus
additional space for file name, additional 24 bytes for second and each
subsequent group linking the event, and additional 32 bytes for each
inotify group for private data. After the conversion inotify event
consumes 48 bytes plus space for file name which is considerably less
memory unless file names are long and there are several groups
interested in the events (both of which are uncommon). Fanotify event
fits in 56 bytes after the conversion (fanotify doesn't care about file
names so its events don't have to have it allocated). A win unless
there are four or more fanotify groups interested in the event.
The conversion also solves the problem with unmount when only inotify is
used as we don't have to grab path references for inotify events.

Record actively mapped pages and provide an api for asserting a given
page is dma inactive before execution proceeds. Placing
debug_dma_assert_idle() in cow_user_page() flagged the violation of the
dma-api in the NET_DMA implementation (see commit 77873803363c "net_dma:
mark broken").
The implementation includes the capability to count, in a limited way,
repeat mappings of the same page that occur without an intervening
unmap. This 'overlap' counter is limited to the few bits of tag space
in a radix tree. This mechanism is added to mitigate false negative
cases where, for example, a page is dma mapped twice and
debug_dma_assert_idle() is called after the page is un-mapped once.

ENTITY_RESET to make sure all resource allocations are reset.

The rport timers must be stopped before the SRP initiator destroys the
resources associated with the SCSI host. This is necessary because
otherwise the callback functions invoked from the SRP transport layer
could trigger a use-after-free. Stopping the rport timers before
invoking scsi_remove_host() can trigger long delays in the SCSI error
handler if a transport layer failure occurs while scsi_remove_host()
is in progress. Hence move the code for stopping the rport timers from
srp_rport_release() into a new function and invoke that function after
scsi_remove_host() has finished. This patch fixes the following
sporadic kernel crash:

The current behavior of the SRP transport layer when a transport layer
error is encountered is to block SCSI command processing only if
fast_io_fail_tmo != off. The current behavior of the FC transport
layer when a transport layer error is encountered is to block SCSI
command processing no matter which value fast_io_fail_tmo has been set
to. Make the behavior of the SRP transport layer consistent with that
of the FC transport layer to avoid confusion.

master->transfer_one returns positive value is not a error.
So set ret to 0 when master->transfer_one returns positive value.
Otherwise, I hit "spi_master spi0: failed to transfer one message from queue"
error when my transfer_one callback returns 1.

end up hitting an assert in glibc manifested by:

This is due to the nature of said operations which sets and clears
the PID. "In the successful one I can see that the page table of
the parent process has been updated successfully to use a
different physical page, so the write of the tid on
that page only affects the child...
On the other hand, in the failed case, the write seems to happen before
the copy of the original page is done, so both the parent and the child
end up with the same value (because the parent copies the page after
the write of the child tid has already happened)."
(Roger's analysis). The nature of this is due to the Xen's commit
of 51e2cac257ec8b4080d89f0855c498cbbd76a5e5
"x86/pvh: set only minimal cr0 and cr4 flags in order to use paging"
the CR0_WP was removed so COW features of the Linux kernel were not
operating properly.
While doing that also update the rest of the CR0 flags to be inline
with what a baremetal Linux kernel would set them to.
In 'secondary_startup_64' (baremetal Linux) sets:

The documentation for spi_master.set_cs() says:
assert or deassert chip select, true to assert
i.e. its "enable" parameter uses assertion-level logic.
This does not match the implementation of spi_set_cs(), which calls
spi_master.set_cs() with the wanted logic level of the chip select line,
which depends on the polarity of the chip select signal.
Correct the documentation to match the implementation.

"A big set this merge window, as we have much going on in this
subsystem. The changes to other subsystems (notably a slew of ARM
machines as I am doing away with their custom APIs) have all been
ACKed to the extent possible.
Major changes this time:
- Some core improvements and cleanups to the new GPIO descriptor API.
This seems to be working now so we can start the exodus to this
API, moving gradually away from the global GPIO numberspace.
- Incremental improvements to the ACPI GPIO core, and move the few
GPIO ACPI clients we have to the GPIO descriptor API right *now*
before we go any further. We actually managed to contain this
*before* we started to litter the kernel with yet another hackish
global numberspace for the ACPI GPIOs, which is a big win.
- The RFkill GPIO driver and all platforms using it have been
migrated to use the GPIO descriptors rather than fixed number
assignments. Tegra machine has been migrated as part of this.
- New drivers for MOXA ART, Xtensa GPIO32 and SMSC SCH311x. Those
should be really good examples of how I expect a nice GPIO driver
to look these days.
- Do away with custom GPIO implementations on a major part of the ARM
machines: ks8695, lpc32xx, mv78xx0. Make a first step towards the
same in the horribly convoluted Samsung S3C include forest. We
expect to continue to clean this up as we move forward.
- Flag GPIO lines used for IRQ on adnp, bcm-kona, em, intel-mid and
lynxpoint.
This makes the GPIOlib core aware that a certain GPIO line is used
for IRQs and can then enforce some semantics such as disallowing a
GPIO line marked as in use for IRQ to be switched to output mode.
- Drop all use of irq_set_chip_and_handler_name(). The name provided
in these cases were just unhelpful tags like "mux" or "demux".
- Extend the MCP23s08 driver to handle interrupts.
- Minor incremental improvements for rcar, lynxpoint, em 74x164 and
msm drivers.
- Some non-urgent bug fixes here and there, duplicate #includes and
that usual kind of cleanups"
Fix up broken Kconfig file manually to make this all compile.

This bug was introduced in commit 7e664b3dec431e ("dm space map metadata:
fix extending the space map").
When extending a dm-thin metadata volume we:
- Switch the space map into a simple bootstrap mode, which allocates
all space linearly from the newly added space.
- Add new bitmap entries for the new space
- Increment the reference counts for those newly allocated bitmap
entries
- Commit changes to disk
- Switch back out of bootstrap mode.
But, the disk commit may allocate space itself, if so this fact will be
lost when switching out of bootstrap mode.
The bug exhibited itself as an error when the bitmap_root, with an
erroneous ref count of 0, was subsequently decremented as part of a
later disk commit. This would cause the disk commit to fail, and thinp
to enter read_only mode. The metadata was not damaged (thin_check
passed).
The fix is to put the increments + commit into a loop, running until
the commit has not allocated extra space. In practise this loop only
runs twice.
With this fix the following device mapper testsuite test passes:
dmtest run --suite thin-provisioning -n thin_remove_works_after_resize

Although we allow recovery in this case, this is not supposed to be
the normal path for hotplugging a CPU back in. This path only exists
to serve those rare platforms where it's not possible to power down
the CPU or reset the CPU. This patch causes the kernel to print a
message when a platform uses this path.

If the kernel is loaded higher in physical memory than normal, and we
calculate PHYS_OFFSET higher than the start of RAM, this leads to
boot problems as we attempt to map part of this RAM into userspace.
Rather than struggle with this, just truncate the mapping.

has unmet direct dependencies (!ZBOOT_ROM)
This is because it's possible to have ZBOOT_ROM enabled, but at the
same time have another option enabled which selects AUTO_ZRELADDR
overriding the !ZBOOT_ROM dependency. Fix this by reversing the
dependencies between ZBOOT_ROM and the options which depend on
!ZBOOT_ROM.

Gaurav reported that perf cannot profile JIT program if it executes the
code on heap. This was because current map__new() only handle JIT on
anon mappings - extends it to handle no_dso (heap, stack) case too.
This patch assumes JIT profiling only provides dynamic function symbols
so check the mapping type to distinguish the case. It'd provide no
symbols for data mapping - if we need to support symbols on data
mappings later it should be changed.

AIO had a missing get, which led to an ioctx leak - after percpu_ref_kill() the
ref was 0 so percpu_ref_put() never saw it hit 0.
This wasn't noticed at the time because it all happened completely silently,
this adds a WARN() which would've caught the aio bug.

pm_runtime_get/put_sync() can sleep so don't hold spinlock while
calling them.
This patch prevents a BUG() during system suspend when
CONFIG_DEBUG_ATOMIC_SLEEP is enabled.
Bug is present in Kernel versions v3.9 onwards.

The MUIC block in max77693 has different I2C address than PMIC. The
driver allocated two regmaps: for PMIC and MUIC. However it used the
same regmap_config (with max_register field) for both regmaps. Actual
maximum address of register for MUIC is different than for PMIC.
Define another regmap_config for MUIC with proper max_register value.

The regmap used by max77686 MFD driver was not freed with regmap_exit()
on driver exit. This lead to leak of resources.
Replace regmap_init_i2c() call in driver probe with initialization of
managed register map so the regmap will be properly freed by the device
management code.
