Currently dev/mem for s390 provides only real memory access. This means
that the CPU prefix pages are swapped. The prefix swap for real memory
works as follows:
Each CPU owns a prefix register that points to a page aligned memory
location "P". If this CPU accesses the address range [0,0x1fff], it is
translated by the hardware to [P,P+0x1fff]. Accordingly if this CPU
accesses the address range [P,P+0x1fff], it is translated by the hardware
to [0,0x1fff]. Therefore, if [P,P+0x1fff] or [0,0x1fff] is read from
the current /dev/mem device, the incorrectly swapped memory content is
returned.
With this patch the /dev/mem architecture code is modified to provide
absolute memory access. This is done via the arch specific functions
xlate_dev_mem_ptr() and unxlate_dev_mem_ptr(). For swapped pages on
s390 the function xlate_dev_mem_ptr() now returns a new buffer with a
copy of the requested absolute memory. In case the buffer was allocated,
the unxlate_dev_mem_ptr() function frees it after /dev/mem code has
called copy_to_user().

wait_cons_dev waits for a particular subchannel to complete an I/O.
It is not necessary to use tpi to get the subchannel id as it is
already known. This avoids changes to the interrupt subclass mask
and allows to remove the lock & unlock of the subchannel lock.

Various improvements of qdio error reporting to the
upper-layer drivers (qeth, zfcp):
- Split QDIO_ERROR_ACTIVATE_CHECK_CONDITION into:
QDIO_ERROR_ACTIVATE: qdio termination interrupt
QDIO_ERROR_GET_BUF_STATE: QIOASSIST eqbs error
QDIO_ERROR_SET_BUF_STATE: QIOASSIST sqbs error
Add QDIO_ERROR_FATAL / QDIO_ERROR_TEMPORARY masks
to ease recovery decision in upper-layer drivers.
- Don't (ab-)use qdio handler errors as return codes
for do_QDIO but use standard error codes:
-ENOBUFS: temporary target CC=2 condition
-EBUSY: unresolved SIGA-W CC=2 busy condition
-EIO: I/O error (CC=1, CC=3)
- Remove unneeded memory clobber from SIGA-R
- Remove EX_TABLE entry on SIGA-W, we want to see these errors

Gadget driver
Add FunctionFS support.
It allows certain USB functions to be provided from userspace, e.g. MTP,
PTP, adb.
The functions provided by the gadget itself are enumerated in
/sys/class/ccg0/functions. The functions which can be supplied
from userspace must be enumerated in /sys/class/ccg0/f_fs/user_functions.
No other userspace functions can be used than specified in the above mentioned
file, but just specifying them there is not enough to activate them.
The userspace functions in order to be activated need also be enumerated
in /sys/class/ccg0/functions.
An example sequence of operations can be as follows:

The above example declares that adb, mtp and ptp functions can be supplied
from userspace through FunctionFS. But of them all only ptp is actually
activated, together with mass_storage, the latter being implemented in
the gadget itself (in kernel, not in userspace). The list of functions
can be modified at runtime while the gadget is not enabled, that is,
after
$ echo 0 > /sys/class/ccg_usb/ccg0/enable
The FunctionFS function is implicitly handled by the gadget, that is,
if a userspace function name is provided in
/sys/class/ccg_usb/ccg0/functions, then the FunctionFS function
is activated.

The Configurable Gadget driver is a composite driver that allows
userspace to change at runtime the list of functions enabled in
its configuration and to configure these functions. It supports
multiple functions: acm, rndis, and mass storage.
It is usually controlled by a daemon that changes the configuration
based on user settings. For example, rndis is enabled when the user
enables sharing the phone data connection.
As an example on how to use it, the following shell commands will
make the gadget disconnect from the host and make it be re-enumerated
as a composite with 1 rndis and 2 acm interfaces, and a different
product id:

echo rndis,acm > /sys/class/ccg_usb/ccg0/functions
echo 2 > /sys/class/ccg_usb/ccg0/f_acm/instances

The driver requires a gadget controller that supports software
control of the D+ pullup and the controller driver must support
disabling the pullup during composite_bind.

The patch 5a6506f (Update at91_udc to use usb_endpoint_descriptor inside the

Commit da0af6e ("usb: Bind devices to ACPI devices when possible") really
tries to force-bind devices even when impossible, unlike what it says in
the subject.
CONFIG_ACPI is not an indication that ACPI tables are actually present, nor
is an indication that any USB relevant information is present in them. There
is no reason to fail the creation of a USB bus if it can't bind it to
ACPI device during initialization.
On systems with CONFIG_ACPI set but without ACPI tables it would cause a
boot panic.

Due to an errata, the PA7300LC generates a TLB miss interruption even on the
prefetch instruction. This means that prefetch(NULL), which is supposed to be
a nop on linux actually generates a NULL deref fault. Fix this by testing the
address of prefetch against NULL before doing the prefetch.

As pointed out by serveral people, PA1.1 only has a type 26 instruction
meaning that the space register must be explicitly encoded. Not giving an
explicit space means that the compiler uses the type 24 version which is PA2.0
only resulting in an illegal instruction crash.

CROSS_COMPILE must be setup before using e.g. cc-option (and a few other
as-*, cc-*, ld-* macros), else they will check against the wrong compiler
when cross-compiling, and may invoke the cross compiler with wrong or
suboptimal compiler options.

This patch changes function gfs2_adjust_quota so that it properly
returns a good (zero) return code on the normal path through the code.
Without this, mounting GFS2 with -o quota=account periodically gave
this error message: GFS2: fsid=cluster:fs: gfs2_quotad: sync error -5

Commit 4924082 "ASoC: core: Flip master for CODECs in the CPU slot of a
CODEC<->CODEC link" added code that was conditional on there being no
PCM/DMA driver for the link. However, it failed to cover the case where
the link was instantiated from device tree, and hence was specified by
DT node rather than name.
This prevents the following error on Toshiba AC100:

Johan Hedberg went to some length justifyng the inclusion of these two
Bluetooth fixes:
"The device_connected fix should be quite self-explanatory, but it's
actually a wider issue than just for keyboards. All profiles that do
incoming connection authorization (e.g. headsets) will break without it
with specific hardware. The reason it wasn't caught earlier is that it
only occurs with specific Bluetooth adapters.
As for the security level patch, this fixes L2CAP socket based security
level elevation during a connection. The HID profile needs this (for
keyboards) and it is the only way to achieve the security level
elevation when using the management interface to talk to the kernel
(hence the management enabling patch being the one that exposes this"
The rtlwifi fix addresses a regression related to firmware loading,
as described in kernel.org bug 43187. It basically just moves a hunk
of code to a more appropriate place.

Currently the slave and master resources are deleted after master freed
all bitmaps. If any resources were not properly cleaned up during the
shutdown process, an Oops would result.
Fix so that delete slave (only) resources during cleanup. Master resources
are cleaned up during unload process, and need not separately be cleaned.
Note that during cleanup, we need to split the resource-tracker freeing
functionality.
Before removing all the bitmaps, we free any leftover slave resources.
However, we can only remove the resource tracker linked list after
all bitmap frees, since some of the freeing functions (e.g.,
mlx4_cleanup_eq_table) use paravirtualized FW commands which expect
the resource tracker linked list to be present.

Consider the following scenario: 2 HCAs, where only one of which can run SRIOV.
If we reset the module parameter, all the VFs of the SRIOV HCA will be
claimed by the PPF host (-- the code relies on num_vfs being non-zero
to avoid this claiming, and num_vfs was reset when pci_enable_sriov failed
for the non-SRIOV HCA).
The solution is not to touch the num_vfs parameter.
Also, eliminate the unneeded check of num_vfs when disabling sriov
(the dev flag bit is sufficient).
