Consolidate the various helpers into a single blk_flush_plug helper that
takes a plk_plug and the from_scheduler bool and switch all callsites to
call it directly. Checks that the plug is non-NULL must be performed by
the caller, something that most already do anyway.

Replace the call to blk_flush_plug_list in blk_mq_submit_bio with a
direct call to blk_mq_flush_plug_list. This means we do not flush
plug callback from stackable devices, which doesn't really help with
the accumulated requests anyway, and it also means the cached requests
aren't freed here as they can still be used later on.

io_unprep_linked_timeout() is broken, first it needs to return back
REQ_F_ARM_LTIMEOUT, so the linked timeout is enqueued and disarmed. But
now we refcounted it, and linked timeouts may get not executed at all,
leaking a request.
Just kill the unprep optimisation.

Currently, IORING_REGISTER_IOWQ_MAX_WORKERS applies only to the task
that issued it, it's unexpected for users. If one task creates a ring,
limits workers and then passes it to another task the limit won't be
applied to the other task.
Another pitfall is that a task should either create a ring or submit at
least one request for IORING_REGISTER_IOWQ_MAX_WORKERS to work at all,
furher complicating the picture.
Change the API, save the limits and apply to all future users. Note, it
should be done first before giving away the ring or submitting new
requests otherwise the result is not guaranteed.

Setting cred->ucounts in cred_alloc_blank does not make sense. The
uid and user_ns are deliberately not set in cred_alloc_blank but
instead the setting is delayed until key_change_session_keyring.
So move dealing with ucounts into key_change_session_keyring as well.
Unfortunately that movement of get_ucounts adds a new failure mode to
key_change_session_keyring. I do not see anything stopping the parent
process from calling setuid and changing the relevant part of it's
cred while keyctl_session_to_parent is running making it fundamentally
necessary to call get_ucounts in key_change_session_keyring. Which
means that the new failure mode cannot be avoided.
A failure of key_change_session_keyring results in a single threaded
parent keeping it's existing credentials. Which results in the parent
process not being able to access the session keyring and whichever
keys are in the new keyring.
Further get_ucounts is only expected to fail if the number of bits in
the refernece count for the structure is too few.
Since the code has no other way to report the failure of get_ucounts
and because such failures are not expected to be common add a WARN_ONCE
to report this problem to userspace.
Between the WARN_ONCE and the parent process not having access to
the keys in the new session keyring I expect any failure of get_ucounts
will be noticed and reported and we can find another way to handle this
condition. (Possibly by just making ucounts->count an atomic_long_t).

The vui parameters are optional. However, the vui data allows to specify
the color space of the encoded video. Write the vui parameters to make
sure that decoders are able to pick up the correct color space.
Also implement the necessary lookup functions to convert the values from
the V4L2 controls to the values specified in the hevc standard.

parameters. Implement it to allow drivers to set the vui parameters in
the coded video stream.

Currently, the driver always writes PAL as video format into the SPS of
the encoded stream.
Set the video format to the default value 5 (unspecified) and use the
color description that is already configured on the channel as color
space. This fixes the color space definition in the coded data to
reflect the configured color space of the video data that is encoded.
Add lookup functions to convert the color primaries, transfer function
and matrix coefficients from the V4L2 control values to the values
specified in the h.264 standard.

The lookup of the h.264 or hevc values for the respective V4L2 controls
is done by the driver that uses the sps/pps generator and not in the
generator. Therefore, it is more intuitive to define these functions
directly in the header and not in the module.
Extract the functions to the headers as static inline functions.
Also simplify the function name and add kernel-doc for the hevc
functions.

The bit rate in the SPS can be scaled by an exponent, which allows to
reduce the number of bits in the SPS in case of high bit rates.
The driver did not scale the bitrate, but used a scaling exponent of 0.
Fix this by properly calculating the scaling factor and writing the bit
rate as value and scaling factor into the SPS.

The VCU allows to specify the QP per frame and coding unit. A buffer
that specifies the QP is passed via the ep2 field in the ENCODE_FRAME
message.
The driver currently does not support the external QP table. Simplify
the driver by not setting the ep2 fields at all.

The fields for the number of rows and columns in the encode frame
response message are switched. This causes broken PPS, if the encoder
uses tiles for encoding and the number of rows and columns differ.
Write the fields of the response message into the correct fields of the
the internal data structure when parsing the response message.

The encoder buffer can have a negative impact on the quality of the
encoded video.
Add a control to allow user space to disable the encoder buffer per
channel if the VCU supports the encoder buffer but the quality is not
sufficient.

The encoder buffer serves as a cache for reference frames during the
encoding process. The encoder buffer significantly reduces the bandwidth
requirement for read accesses on the AXI ports of the VCU, but slightly
reduces the quality of the encoded video.
The encoder buffer must be configured as a whole during the firmware
initialization and later explicitly enabled for every channel that shall
use the encoder buffer.
Prior to firmware version 2019.2, it was necessary to explicitly set the
size of the encoder buffer for every channel. Since 2019.2 it is
sufficient to enable the encoder buffer and leave the rest to the
firmware. Therefore, only support the encoder buffer for firmware 2019.2
and later.

The allegro driver must ensure that the mcu and core clocks are enabled
and set to the expected clock rate before trying to load the firmware
and to reset the MCU.
Up until now, the driver assumed that the clocks are always enabled in
the PL (programming logic), because the xlnx_vcu driver did not export
the clocks to other drivers. This has changed and by explicitly enabling
the clocks in the driver, this assumption can be dropped.
It might even be possible to disable the clocks for the encoder if the
encoder is not used. However, the behavior is not documented and it
might be necessary to reinitialize the encoder after deactivating the
clocks. Play it safe by sticking to the current behavior.

The VCU provides information about its configuration in a dedicated
register space. These settings include, for example, the expected clock
rates and the configuration of the encoder buffer. In the device tree,
the settings are described by the "xlnx,vcu-settings" compatible.
The settings are needed to correctly configure the clocks and the
encoder buffer.
Lookup the VCU settings in the device tree and make it accessible to the
driver via a regmap.

If the module probe finished, but the firmware initialization failed,
removing the module must not revert the firmware initialization.
Add a field to track the status of the firmware initialization and only
roll it back, if the firmware was successfully initialized.

The mailbox is initialized after the interrupt handler is installed. As
the firmware is loaded and started even later, it should not happen that
the interrupt occurs without the mailbox being initialized.
As the Linux Driver Verification project (linuxtesting.org) keeps
reporting this as an error, add a check to ignore interrupts before the
mailbox is initialized to fix this potential null pointer dereference.

Using local kmaps slightly reduces the chances to stray writes, and
the bvec interface cleans up the code a little bit.
Also switch from page_address to bvec_kmap_local for cbv to be on the
safe side and to avoid pointlessly poking into bvec internals.

Calculation of cache_set's cached sectors is done by travelling
cached_devs list as shown below:

c->cached_dev_sectors = sectors;

But cached_dev won't be unlinked from c->cached_devs list until we call
following list_move(&dc->list, &uncached_devices),
so previous fix in 'commit 46010141da6677b81cc77f9b47f8ac62bd1cbfd3
("bcache: recal cached_dev_sectors on detach")' is wrong, now we move
it to its right place.

In register_bcache(), there are several cases we didn't set
correct error info (return value and/or error message):
- if kzalloc() fails, it needs to return ENOMEM and print
"cannot allocate memory";
- if register_cache() fails, it's better to propagate its
return value rather than using default EINVAL.

There sre 3 bits in member high of struct bkey are never used, and no
plan to support them in future,
- HEADER_SIZE, start at bit 58, length 2 bits
- KEY_PINNED, start at bit 55, length 1 bit
No any kernel code, or user space tool references or accesses the three
bits. Therefore it is possible and feasible to reserve the valuable bits
from bkey.high. They can be used in future for other purpose.

This check is meant to catch cases where a requeue is attempted on a
request that is still inserted. It's never really been useful to catch any
misuse, and now it's actively wrong. Outside of that, this should not be a
BUG_ON() to begin with.
Remove the check as it's now causing active harm, as requeue off the plug
path will trigger it even though the request state is just fine.

__dasd_device_check_path_events() calls the discipline path event handler.
This handler can leave the 'to be verified pathmask' populated for an
additional verification.
There is a race window where the worker has finished before
dasd_path_clear_all_verify() is called which resets the tbvpm.
Due to this there could be outstanding path verifications missed.
Fix by clearing the pathmasks before calling the handler and add them
again in case of an error.

dasd_eckd_path_available_action() does a memory allocation to store
the per path configuration data permanently.
In the unlikely case that this allocation fails there is no conf_data
stored for the corresponding path.
This is OK since this is not necessary for an operational path but some
features like control unit initiated reconfiguration (CUIR) do not work.
To fix this add the path to the 'to be verified pathmask' again and
schedule the handler again.

Reproduce this issue as follows:
1. nbd-server 8000 /tmp/disk
2. nbd-client localhost 8000 /dev/nbd1
3. cat /sys/block/nbd1/pid
Then trigger use-after-free in pid_show.
Reason is after do step '2', nbd-client progress is already exit. So
it's task_struct already freed.
To solve this issue, revert part of 6521d39a64b3's modify and remove
useless 'recv_task' member of nbd_device.

percpu_ref_put() are inlined for performance and bloat the binary, we
don't care about the fail case of blk_try_enter_queue(), so we can
replace it with a call to blk_queue_exit().

First, get rid of an extra branch and chain error checks. Also reshuffle
it with bio_advance(), so it goes closer to the final check, with that
the compiler loads rq->rq_flags only once, and also doesn't reload
bio->bi_iter.bi_size if bio_advance() didn't actually advanced the iter.

Convert bdev->bd_disk->queue to bdev_get_queue(), which is faster.
Apparently, there are a few such spots in block that got lost during
rebases.

When posix_clock_register() returns an error, the name allocated
in dev_set_name() will be leaked, the put_device() should be used
to give up the device reference, then the name will be freed in
kobject_cleanup() and other memory will be freed in ptp_clock_release().

Commit f2fb6b6275eb ("net: stmmac: enable timestamp snapshot for required PTP
packets in dwmac v5.10a") already addresses this problem for the dwmac
v5.10. However, same holds true for all dwmacs above version v4.10. Correct the
check accordingly. Afterwards everything works as expected.
Tested on Intel Atom(R) x6414RE Processor.
