In autofs4_d_automount(), if a mount fail occurs the AUTOFS_INF_PENDING
mount pending flag is not cleared.
One effect of this is when using the "browse" option, directory entry
attributes show up with all "?"s due to the incorrect callback and
subsequent failure return (when in fact no callback should be made).

Discourage distros from enabling CONFIG_RCU_USER_QS
because it brings overhead for no benefits yet.
It's not a useful feature on its own until we can
fully run an adaptive tickless kernel.

Missing error types, attributes, and report fields. Pad out
to 64-bytes.
Make string reporting cleaner and easier to extend in the future using
"const char *" arrays that index by either bit position, or absolute
field value.
Report the raw 64-byte error report as a sequence of u64s before the
annotated version.
Only report fields which are valid, given the context and the
attribute bits which are set.
For shutdown requests, use the local copy of the error report not the
one we just freed up back to the queue. Also, use orderly_poweroff()
just like the Domain Services shutdown request code does.
If the real-address reported is "-1" (unknown) try to disassemble the
instruction to report the effective address of the access. Only do
this in privileged mode.

Pull block IO update from Jens Axboe:
"Core block IO bits for 3.7. Not a huge round this time, it contains:
- First series from Kent cleaning up and generalizing bio allocation
and freeing.
- WRITE_SAME support from Martin.
- Mikulas patches to prevent O_DIRECT crashes when someone changes
the block size of a device.
- Make bio_split() work on data-less bio's (like trim/discards).
- A few other minor fixups."
Fixed up silent semantic mis-merge as per Mikulas Patocka and Andrew
Morton. It is due to the VM no longer using a prio-tree (see commit
6b2dbba8b6ac: "mm: replace vma prio_tree with an interval tree").
So make set_blocksize() use mapping_mapped() instead of open-coding the
internal VM knowledge that has changed.

Commit e1aab161e013 ("socket: initial cgroup code.") causes a build
error when CONFIG_INET is disabled in Linus' tree:
net/built-in.o: In function `sk_update_clone':
net/core/sock.c:1336: undefined reference to `sock_update_memcg'
sock_update_memcg() is only defined when CONFIG_INET is enabled, so fix
it by defining the dummy function without this option.

Commit 6b8029fab641 ("rtc: kconfig: remove unnecessary dependencies")
removed various 'depends on RTC_CLASS' dependencies but also removed a
few 'default RTC_CLASS' statements, which actually changed default
behavior.
This resulted in the various RTC interfaces (sysfs, proc, dev) all being
disabled by default, even when RTC_CLASS is enabled:
# CONFIG_RTC_INTF_SYSFS is not set
# CONFIG_RTC_INTF_PROC is not set
# CONFIG_RTC_INTF_DEV is not set
which is different from previous behavior (all of these where enabled.)
To fix, add back the 'default RTC_CLASS' statments to each of the
RTC_INTF_* options.
I noticed this because some RTC tests started failing on my TI OMAP
platforms because /dev/rtc0 was not present anymore, even though the
driver was present and RTC_CLASS was enabled.

kvasprintf
Previously kvasprintf() allocation was being done through kmalloc(),
thus producing an inaccurate trace report.
This is a common problem: in order to get accurate callsite tracing, a
lib/utils function shouldn't allocate kmalloc but instead use
kmalloc_track_caller.

You may notice that DDI_BUF_CTL is enabled in .enable but disabled in
.post_disable instead of .disable. Yes, the mode set sequence is not
exactly symmetrical, but let's assume the spec is correct unless we
can prove it's wrong.

drm/i915: implement WaDisableEarlyCull for VLV and IVB
Notice that the original patch sent to the mailing list did not
include the Haswell chunk, it was added later.
The bit set by the commit does not exist on Haswell machines (at least
that's what the documentation says). Also, the commit gives me a GPU
hang every time we're loading the driver. So let's revert the Haswell
chunk, making the patch do only what its title actually says.

tpm_write calls tpm_transmit without checking the return value and
assigns the return value unconditionally to chip->pending_data, even if
it's an error value.
This causes three bugs.
So if we write to /dev/tpm0 with a tpm_param_size bigger than
TPM_BUFSIZE=0x1000 (e.g. 0x100a)
and a bufsize also bigger than TPM_BUFSIZE (e.g. 0x100a)
tpm_transmit returns -E2BIG which is assigned to chip->pending_data as
-7, but tpm_write returns that TPM_BUFSIZE bytes have been successfully
been written to the TPM, altough this is not true (bug #1).
As we did write more than than TPM_BUFSIZE bytes but tpm_write reports
that only TPM_BUFSIZE bytes have been written the vfs tries to write
the remaining bytes (in this case 10 bytes) to the tpm device driver via
tpm_write which then blocks at

for 60 seconds, since data_pending is -7 and nobody is able to
read it (since tpm_read luckily checks if data_pending is greater than
0) (#bug 2).
After that the remaining bytes are written to the TPM which are
interpreted by the tpm as a normal command. (bug #3)
So if the last bytes of the command stream happen to be a e.g.
tpm_force_clear this gets accidentally sent to the TPM.
This patch fixes all three bugs, by propagating the error code of
tpm_write and returning -E2BIG if the input buffer is too big,
since the response from the tpm for a truncated value is bogus anyway.
Moreover it returns -EBUSY to userspace if there is a response ready to be
read.

The virq_disabled flag tracks the userspace view of INTx masking
across interrupt mode changes, but we're not consistently applying
this to the interrupt and masking handler notion of the device.
Currently if the user sets DisINTx while in MSI or MSIX mode, then
returns to INTx mode (ex. rebooting a qemu guest), the hardware has
DisINTx+, but the management of INTx thinks it's enabled, making it
impossible to actually clear DisINTx. Fix this by updating the
handler state when INTx is re-enabled.

We need to be ready to recieve an interrupt as soon as we call
request_irq, so our eventfd context setting needs to be moved
earlier. Without this, an interrupt from our device or one
sharing the interrupt line can pass a NULL into eventfd_signal
and oops.

Our mmap path mistakely relied on vma->vm_pgoff to get set in
remap_pfn_range. After b3b9c293, that path only applies to
copy-on-write mappings. Set it in our own code.

Problems with the previous code:
- HDMI just uses WRPLL1 for everything, so dual head cases might not
work sometimes.
- At encoder->mode_set we just write the PLL register without doing
any kind of check (e.g., check if the PLL is already being used).
- There is no way to fail and return error codes at
encoder->mode_set.
- We write to PORT_CLK_SEL at mode_set and we never disable it.
- Machines hang due to wrong clock enable/disable sequence.
So here we rewrite the code, making it a little more like the
pre-Haswell PLL mode set code:
- Check PLL availability at ironlake_crtc_mode_set.
- Try to use both WRPLLs.
- Check if PLLs are used before actually trying to use them, and
properly fail with error messages.
- Enable/disable PORT_CLK_SEL at the right place.
- Add some WARNs to check for bugs.
The next improvement will be to try to reuse PLLs if the timings
match, but this is content for another patch and it's already
documented with a TODO comment.

This patch try to fix the S3 regression https://lkml.org/lkml/2012/10/5/433,
which includes below line:
[ 1554.684638] sysfs: cannot create duplicate filename '/devices/pnp0/00:0c/ppi'
The root cause is that ppi sysfs teardown code is MIA, so while S3 resume,
the ppi kobject will be created again upon existing one.
To make the tear down code simple, change the ppi subfolder creation from
using kobject_create_and_add to just using a named ppi attribute_group. Then
ppi sysfs teardown could be done with a simple sysfs_remove_group call.
Adjusted the name & return type for ppi sysfs init function.

It's a copy of ironlake_set_pipeconf with 2 differences:
- There is no BPC field to set.

On ironlake_crtc_mode_set, WARN if not using IBX or CPT.
On haswell_crtc_mode_set, only run IBX/CPT code on IBX/CPT. I am still
not sure whether IBX/CPT will be possible with a Haswell CPU, so leave
the code there for now and put a WARN in case we spot it.

And the right time is exactly after/before changing PIPE_CONF. See the
documentation about the mode set sequence.
This code is not inside any encoder-specific callback because
DDI_FUNC_CTL is part of the pipe, so it is used by all encoders.

Right now, we're trying to enable LCPLL at every mode set, but we're
never disabling it. Also, we really don't want to be disabling LCPLL
since it requires a very complex disable/enable sequence. This
register should really be set by the BIOS and we shouldn't be touching
it. Still, let's try to check its value and print some errors in case
we find something wrong. We're also adding intel_ddi_get_cdclk_freq
which will be used later in other places.

Many hwmon drivers use jiffies but omit the inclusion of the header
file. Fix that, and also fix one driver which was including the header
file but didn't need it.

Git commit 09a1d34f8535ecf9 "nohz: Make idle/iowait counter update
conditional" introduced a bug in regard to cpu hotplug. The effect is
that the number of idle ticks in the cpu summary line in /proc/stat is
still counting ticks for offline cpus.
Reproduction is easy, just start a workload that keeps all cpus busy,
switch off one or more cpus and then watch the idle field in top.
On a dual-core with one cpu 100% busy and one offline cpu you will get
something like this:
%Cpu(s): 48.7 us, 1.3 sy, 0.0 ni, 50.0 id, 0.0 wa, 0.0 hi, 0.0 si,
%0.0 st
The problem is that an offline cpu still has ts->idle_active == 1.
To fix this we should make sure that the cpu is online when calling
get_cpu_idle_time_us and get_cpu_iowait_time_us.
