Two threads can try to fire the irq_sim with different offsets and will
end up fighting for the irq_work asignment. Thomas Gleixner suggested a
solution based on a bitfield where we set a bit for every offset
associated with an interrupt that should be fired and then iterate over
all set bits in the interrupt handler.
This is a slightly modified solution using a bitmap so that we don't
impose a limit on the number of interrupts one can allocate with
irq_sim.

Since the addition of platform MSI support, there were two helpers
supposed to allocate/free IRQs for a device:
platform_msi_domain_alloc_irqs()
platform_msi_domain_free_irqs()
In these helpers, IRQ descriptors are allocated in the "alloc" routine
while they are freed in the "free" one.
Later, two other helpers have been added to handle IRQ domains on top
of MSI domains:
platform_msi_domain_alloc()
platform_msi_domain_free()
Seen from the outside, the logic is pretty close with the former
helpers and people used it with the same logic as before: a
platform_msi_domain_alloc() call should be balanced with a
platform_msi_domain_free() call. While this is probably what was
intended to do, the platform_msi_domain_free() does not remove/free
the IRQ descriptor(s) created/inserted in
platform_msi_domain_alloc().
One effect of such situation is that removing a module that requested
an IRQ will let one orphaned IRQ descriptor (with an allocated MSI
entry) in the device descriptors list. Next time the module will be
inserted back, one will observe that the allocation will happen twice
in the MSI domain, one time for the remaining descriptor, one time for
the new one. It also has the side effect to quickly overshoot the
maximum number of allocated MSI and then prevent any module requesting
an interrupt in the same domain to be inserted anymore.
This situation has been met with loops of insertion/removal of the
mvpp2.ko module (requesting 15 MSIs each time).

TXQ SQ closure is followed by closing the corresponding CQ. A pending
DIM work would try to modify the now non-existing CQ.
This would trigger an error:

Currently, we are deleting offloaded encap flows in case the relevant neigh
becomes unconnected while the encap is valid (a sign that it used to be
connected), or if the curr neigh mac is different from the cached mac
(a sign that the remote side changed their mac).
The 2nd check also applies when the neigh becomes connected on the 1st
time (we start with zero mac). Before the offending commit, the deleting
handler was practically no op, as no flows were offloaded. But since
that commit, we offload neigh-less encap flows to slow path.
Under mirroring scheme, we go into the delete handler, attempt to unoffload a
mirror rule which was never set (as we were offloading to slow path) and crash.
Fix that by calling the delete handler only when the encap is valid,
which covers both cases mentioned above.

When a neighbour is resolved, we delete the goto slow path rule from HW.
The eswitch flow attributes where not properly initialized on that case,
hence we mess up the eswitch refcounts for chain zero (the default one).
Fix that along with making sure to use semicolons and not commas on that code;

Add check of MPWQE stride size is within range supported by HW. In case

The cap bits locations for the fdb caps of multi path to table (used for
local mirroring) and multi encap (used for prio/chains) were wrongly used
in swapped locations. This went unnoted so far b/c we tested the offending
patch with CX5 FW that supports both of them. On different environments where
not both caps are supported, we will be messed up, fix that.

This argument will specify how many I/O queues will be connected in
create_ctrl in addition to nr_io_queues. With this configuration, I/O
that carries payload from the host to the target, will use the default
hctx queue map, and I/O that involves target to host transfers will use
the read hctx queue map.

This patch adds necessary fields in the target data structures to
support error log page. For a target controller, we add a new error log
field to maintain the error log, at any given point we maintain error
entries equal to NVMET_ERROR_LOG_SLOTS for each controller. In the
following patch, we also update the error log page entry in the I/O
completion path so we introduce a spinlock for synchronization of the
log.
For nvmet_req, we add a new field error_loc to hold the location of
the error in the command when the actual error occurs for each request
and a starting LBA if applicable.

When boxes are run near (or to) OOM, we have a problem with the discard
page allocation in nvme. If we fail allocating the special page, we
return busy, and it'll get retried. But since ordering is honored for
dispatch requests, we can keep retrying this same IO and failing. Behind
that IO could be requests that want to free memory, but they never get
the chance.
Allocate a fixed discard page per controller for a safe fallback, and use
that if the initial allocation fails.

This patch implements the NVMe over TCP host driver. It can be used to
connect to remote NVMe over Fabrics subsystems over good old TCP/IP.
The driver implements the TP 8000 of how nvme over fabrics capsules and
data are encapsulated in nvme-tcp pdus and exchaged on top of a TCP byte
stream. nvme-tcp header and data digest are supported as well.
To connect to all NVMe over Fabrics controllers reachable on a given taget
port over TCP use the following command:
nvme connect-all -t tcp -a $IPADDR
