This patch prepares the iscsi-target login code for multi-plexing
support. This includes:
- Adding iscsi_tpg_np->tpg_np_kref + iscsit_login_kref_put() for
handling callback of iscsi_tpg_np->tpg_np_comp
- Adding kref_put() in iscsit_deaccess_np()
- Adding kref_put() and wait_for_completion() in
iscsit_reset_np_thread()
- Refactor login failure path release logic into
iscsi_target_login_sess_out()
- Update __iscsi_target_login_thread() to handle
iscsi_post_login_handler() asynchronous completion
- Add shutdown parameter for iscsit_clear_tpg_np_login_thread*()

In the current ACPIPHP notify handler we always go directly for a
rescan of the parent bus if we get a device check notification for
a device that is not a bridge. However, this obviously is
overzealous if nothing really changes, because this way we may rescan
the whole PCI hierarchy pretty much in vain.
That happens on Alex Williamson's machine whose ACPI tables contain
device objects that are supposed to coresspond to PCIe root ports,
but those ports aren't physically present (or at least they aren't
visible in the PCI config space to us). The BIOS generates multiple
device check notifies for those objects during boot and for each of
them we go straight for the parent bus rescan, but the parent bus is
the root bus in this particular case. In consequence, we rescan the
whole PCI bus from the top several times in a row, which is
completely unnecessary, increases boot time by 50% (after previous
fixes) and generates excess dmesg output from the PCI subsystem.
Fix the problem by checking if we can find anything new in the
slot corresponding to the device we've got a device check notify
for and doing nothig if that's not the case.
The spec (ACPI 5.0, Section 5.6.6) appears to mandate this behavior,
as it says:
Device Check. Used to notify OSPM that the device either appeared
or disappeared. If the device has appeared, OSPM will re-enumerate
from the parent. If the device has disappeared, OSPM will
invalidate the state of the device. OSPM may optimize out
re-enumeration.
Therefore, according to the spec, we are free to do nothing if
nothing changes.

rt2800: rearrange bbp/rfcsr initialization
make Maxim machine freeze when try to start wireless device.
Initialization order and sending MCU_BOOT_SIGNAL request, changed in
above commit, is important. Doing things incorrectly make PCIe bus
problems, which can froze the machine.
This patch change initialization sequence like vendor driver do:
function NICInitializeAsic() from
2011_1007_RT5390_RT5392_Linux_STA_V2.5.0.3_DPO (PCI devices) and
DPO_RT5572_LinuxSTA_2.6.1.3_20121022 (according Mediatek, latest driver
for RT8070/RT3070/RT3370/RT3572/RT5370/RT5372/RT5572 USB devices).
It fixes freezes on Maxim system.

On most 64 Bit systems unsigned long is 64 bit long and then -MAX_ERRNO
is out of the range of a u32 used to store the error code in.
This patch casts the -MAX_ERRNO to a u32 instead.
This fixes a regression introduced in:

In case of __dev_alloc_skb() failure rtl8187_init_urbs()
calls usb_free_urb(entry) where 'entry' can points to urb
allocated at the previous iteration. That means refcnt will be
decremented incorrectly and the urb can be used after memory
deallocation.
The patch fixes the issue and implements error handling of init_urbs
in rtl8187_start().
Found by Linux Driver Verification project (linuxtesting.org).

The cw1200_spi driver tries to mirror the cw1200_sdio driver's lock
API, which relies on sdio_claim_host/sdio_release_host to serialize
hardware operations across multiple threads.
Unfortunately the implementation was flawed, as it lacked a way to wake
up the lock requestor when there was contention, often resulting in a
hang.
This problem was uncovered while trying to fix the
spi-transfers-in-interrupt-context BUG() corrected in the previous
patch. Many thanks to Dave Sizeburns for his assistance in fixing this.

When we get an interrupt from the hardware, the first thing the driver does
is tell the device to mask off the interrupt line. Unfortunately this
involves a SPI transaction in interrupt context. Some (most?) SPI
controllers perform the transfer asynchronously and try to sleep.
This is bad, and triggers a BUG().
So, work around this by using adding a hwbus hook for the cw1200 driver
core to call. The cw1200_spi driver translates this into
irq_disable()/irq_enable() calls instead, which can safely be called in
interrupt context.
Apparently the platforms I used to develop the cw1200_spi driver used
synchronous spi_sync() implementations, which is why this didn't surface
until now.

rbd_snap_name() calls rbd_dev_v{1,2}_snap_name() depending on the
format of the image. The format 1 version returns NULL on error, which
is handled by the caller. The format 2 version returns an ERR_PTR,
which the caller of rbd_snap_name() does not expect.
Fortunately this is unlikely to occur in practice because
rbd_snap_id_by_name() is called before rbd_snap_name(). This would hit
similar errors to rbd_snap_name() (like the snapshot not existing) and
return early, so rbd_snap_name() would not hit an error unless the
snapshot was removed between the two calls or memory was exhausted.
Use an ERR_PTR in rbd_dev_v1_snap_name() so that the specific error
can be propagated, and it is consistent with rbd_dev_v2_snap_name().
Handle the ERR_PTR in the only rbd_snap_name() caller.

This prevents erroring out while adding a device when a snapshot
unrelated to the current mapping is deleted between reading the
snapshot context and reading the snapshot names. If the mapped
snapshot name is not found an error still occurs as usual.

Removing a device deallocates the disk, unschedules the watch, and
finally cleans up the rbd_dev structure. rbd_dev_refresh(), called
from the watch callback, updates the disk size and rbd_dev
structure. With no locking between them, rbd_dev_refresh() may use the
device or rbd_dev after they've been freed.
To fix this, check whether RBD_DEV_FLAG_REMOVING is set before
updating the disk size in rbd_dev_refresh(). In order to prevent a
race where rbd_dev_refresh() is already revalidating the disk when
rbd_remove() is called, move the call to rbd_bus_del_dev() after the
watch is unregistered and all notifies are complete. It's safe to
defer deleting this structure because no new requests can be submitted
once the RBD_DEV_FLAG_REMOVING is set, since the device cannot be
opened.

The only user of rbd_obj_notify_ack() is rbd_watch_cb(). It used
asynchronously with no tracking of when the notify ack completes, so
it may still be in progress when the osd_client is shut down. This
results in a BUG() since the osd client assumes no requests are in
flight when it stops. Since all notifies are flushed before the
osd_client is stopped, waiting for the notify ack to complete before
returning from the watch callback ensures there are no notify acks in
flight during shutdown.
Rename rbd_obj_notify_ack() to rbd_obj_notify_ack_sync() to reflect
its new synchronous nature.

To ensure rbd_dev is not used after it's released, flush all pending
notify callbacks before calling rbd_dev_image_release(). No new
notifies can be added to the queue at this point because the watch has
already be unregistered with the osd_client.

"VFIO updates include safer default file flags for VFIO device fds, an
external user interface exported to allow other modules to hold
references to VFIO groups, a fix to test for extended config space on
PCIe and PCI-x, and new hot reset interfaces for PCI devices which
allows the user to do PCI bus/slot resets when all of the devices
affected by the reset are owned by the user.
For this last feature, the PCI bus reset interface, I depend on
changes already merged from Bjorn's PCI pull request. I therefore
merged my tree up to commit cb3e433, which I think was the correct
action, but as Stephen Rothwell noted, I failed to provide a commit

Add error reporting to of_amba_device_create() so the user knows
when (and why) some device tree nodes fail to initialize.
[ The issue was spotted on Universal C210 board (using revision 0 of
ARM Exynos4210 SoC) on which initialization was silently failing
for PL330 MDMA1 device tree node (it was using the wrong addres
resulting in amba_device_add() returning -ENODEV). ]

My g33 here seems to be shockingly good at hitting them all. This time
around kms_flip/flip-vs-panning-vs-hang blows up:
intel_crtc_wait_for_pending_flips correctly checks for gpu hangs and
if a gpu hang is pending aborts the wait for outstanding flips so that
the setcrtc call will succeed and release the crtc mutex. And the gpu
hang handler needs that lock in intel_display_handle_reset to be able
to complete outstanding flips.
The problem is that we can race in two ways:
- Waiters on the dev_priv->pending_flip_queue aren't woken up after
we've the reset as pending, but before we actually start the reset
work. This means that the waiter doesn't notice the pending reset
and hence will keep on hogging the locks.
Like with dev->struct_mutex and the ring->irq_queue wait queues we
there need to wake up everyone that potentially holds a lock which
the reset handler needs.
- intel_display_handle_reset was called _after_ we've already
signalled the completion of the reset work. Which means a waiter
could sneak in, grab the lock and never release it (since the
pageflips won't ever get released).
Similar to resetting the gem state all the reset work must complete
before we update the reset counter. Contrary to the gem reset we
don't need to have a second explicit wake up call since that will
have happened already when completing the pageflips. We also don't
have any issues that the completion happens while the reset state is
still pending - wait_for_pending_flips is only there to ensure we
display the right frame. After a gpu hang&reset events such
guarantees are out the window anyway. This is in contrast to the gem
code where too-early wake-up would result in unnecessary restarting
of ioctls.
Also, since we've gotten these various deadlocks and ordering
constraints wrong so often throw copious amounts of comments at the
code.
This deadlock regression has been introduced in the commit which added
the pageflip reset logic to the gpu hang work:

The virtio_pci_freeze/restore are defined under CONFIG_PM but is used
by SET_SYSTEM_SLEEP_PM_OPS macro, which is defined under
CONFIG_PM_SLEEP. So if CONFIG_PM_SLEEP is not cofigured but
CONFIG_PM_RUNTIME is, the following warning message appeared:

Detangle the additional state of whether or not the hw has the pfit
enabled from whether it has zero size. This allows us to cleanly
distinguish in the code when we expect the pfit to be enabled (for
Haswell pc8), and when the BIOS is confused and needs sanitizing.

If we have multiple sessions on a system, we normally don't want
background sessions to read input events. Otherwise, it could capture
passwords and more entered by the user on the foreground session. This is
a real world problem as the recent XMir development showed:

We currently rely on sessions to release input devices when being
deactivated. This relies on trust across sessions. But that's not given on
usual systems. We therefore need a way to control which processes have
access to input devices.
With VTs the kernel simply routed them through the active /dev/ttyX. This
is not possible with evdev devices, though. Moreover, we want to avoid
routing input-devices through some dispatcher-daemon in userspace (which
would add some latency).
This patch introduces EVIOCREVOKE. If called on an evdev fd, this revokes
device-access irrecoverably for that *single* open-file. Hence, once you
call EVIOCREVOKE on any dup()ed fd, all fds for that open-file will be
rather useless now (but still valid compared to close()!). This allows us
to pass fds directly to session-processes from a trusted source. The
source keeps a dup()ed fd and revokes access once the session-process is
no longer active.
Compared to the EVIOCMUTE proposal, we can avoid the CAP_SYS_ADMIN
restriction now as there is no way to revive the fd again. Hence, a user
is free to call EVIOCREVOKE themself to kill the fd.
Additionally, this ioctl allows multi-layer access-control (again compared
to EVIOCMUTE which was limited to one layer via CAP_SYS_ADMIN). A middle
layer can simply request a new open-file from the layer above and pass it
to the layer below. Now each layer can call EVIOCREVOKE on the fds to
revoke access for all layers below, at the expense of one fd per layer.
There's already ongoing experimental user-space work which demonstrates
how it can be used:

The extra new ABS_xx values resulted in ABS_MAX no longer being a
power-of-two, which broke the comparison logic. It also caused the
ioctl numbers to overflow into the next byte, causing problems for that.
We'll try again for 3.13.

Let's get rid of another sparse false positive:
drivers/s390/crypto/ap_bus.c:416:64: warning:
cast truncates bits from constant value (102030405060708 becomes 5060708)
So instead of using a cast let's use an and-mask.
That way sparse remains silent and one doesn't always have to check
if this is a valid warning/bug or just a false positive.

The spec suggests that we should use _OST to notify the platform
about the status of notifications it sends us, for example so that
it doesn't repeate a notification that has been handled already.
This turns out to help reduce the amount of diagnostic output from
the ACPIPHP subsystem and speed up boot on at least one system that
generates multiple device check notifies for PCIe devices on the root
bus during boot.

Sometimes we may get a spurious device check or bus check notify for
a hotplug device and in those cases we should avoid doing all of the
configuration work needed when something actually changes. To that
end, check the return value of pci_scan_slot() in enable_slot() and
bail out early if it is 0.
This turns out to help reduce the amount of diagnostic output from
the ACPIPHP subsystem and speed up boot on at least one system that
generates multiple device check notifies for PCIe devices on the root
bus during boot.

UIC attributes can be set with using DME_SET command for
power mode change. For configuration the link capability
attributes are used, which is updated after successful
link startup.

Setting PA_PWRMode using DME_SET triggers the power mode
change. And then the result will be given by the HCS.UPMCRS.
This operation should be done atomically.

IACTH(Interrupt aggregation counter threshold) value is allowed
up to 0x1F and current setting value is the maximum.
This value is related with NUTRS(max:0x20) of HCI's capability.
Considering HCI controller doesn't support the maximum, IACTH
setting should be adjusted with possible value.
For that, existing 'ufshcd_config_int_aggr' is split into two part
[reset, configure].

Unlike 'GOOD' and 'CHECK CONDITION', other status values in
Response UPIU may or may not contain sense data. That is returning
sense data isn't obvious. So, in this case the Data Segment Length
field should be checked. If a non-zero value, it means that UPIU
has Sense Data in the Data Segment area.
