This patch moves regs-usb-hsotg-phy header file into mach-s3c64xx.
Because it is not used for others except mach-s3c64xx.

The definitions have moved to include/linux/usb/samsung-usb-phy.h,
and plat/usb-phy.h is unavailable from drivers in a multiplatform
configuration.
Also fix up the plat/usb-phy.h header file to use the definitions
from the new header instead of providing a separate copy.

This patch supports to control usb otg phy of S3C64XX. Currently, the
driver for usb otg controls usb otg phy but it can be removed by this
patch.
