Add all the necessary platform code to initialize the dwc3
USB host controller. This code initializes the clocks and
performs a reset on the USB core and PHYs. The driver code
in 'drivers/usb/dwc3' is where the real driver lives.

octeon-platform.c can not be built as a module for two reasons:
(a) the Makefile doesn't allow it:
obj-y := cpu.o setup.o octeon-platform.o octeon-irq.o csrc-octeon.o
(b) the multiple *_initcall() statements, each of which are translated
to a module_init() call when attempting a module build, become
aliases to init_module(). Having more than one alias will cause a
build error.
Hence, rather than adding a linux/module.h include, remove the redundant
MODULE_*() from this file.

Commit 44a7185c2ae6 ("of/platform: Add common method to populate
default bus") added new arch_initcall of_platform_default_populate_init()
that will override device_initcall octeon_publish_devices(). This broke
many OCTEON boards as important devices are not getting probed anymore
(e.g. on EdgeRouter Lite the USB mass storage/rootfs is missing).
Fix by changing octeon_publish_devices() to arch_initcall.

At boot time, do a better job of resetting the USB host controller
to make the frequency "eye" diagram more compliant with the USB
standard while making the controller more reliable.

ehci-octeon driver used a 64-bit dma_mask. With removal of ehci-octeon
and usage of ehci-platform ehci dma_mask is now limited to 32 bits
(coerced in ehci_platform_probe).
Provide a flag in ehci platform data to allow use of 64 bits for
dma_mask.

Instead rely on device tree information for ehci and ohci.

The boot hangs early on EBH5600 board when octeon_fdt_pip_iface() is
trying enumerate a non-existant interface. The actual hang happens in
cvmx_helper_interface_get_mode():
mode.u64 = cvmx_read_csr(CVMX_GMXX_INF_MODE(interface));
when interface == 4. We can avoid this situation by first checking that
the interface exists in the DTB.

During the pruning of the device tree octeon_fdt_pip_iface() is called
for each PIP interface and every port up to the port count is removed
from the device tree. However, the count was set to the return value of
cvmx_helper_interface_enumerate() which doesn't actually return the
count but just returns zero on success. This effectively removed *all*
ports from the tree.
Use cvmx_helper_ports_on_interface() instead to fix this. This
successfully restores the 3 ports of my ERLite-3 and fixes the "kernel
assigns random MAC addresses" issue.

The patch needs to eliminate the definition of OCTEON_IRQ_BOOTDMA so
that the device tree code can map the interrupt, so in order to not
temporarily break things, we do a single patch to both the interrupt
registration code and the pata_octeon_cf driver.
Also rolled in is a conversion to use hrtimers and corrections to the
timing calculations.

The device tree will supply the register bank base addresses, make
register addressing relative to those. PHY connection is now
described by the device tree.
The OCTEON_IRQ_MII{0,1} symbols are also removed as they are now
unused and interfere with the irq_domain used for device tree irq
mapping.

There are three parts to this:
1) Remove the definitions of OCTEON_IRQ_TWSI and OCTEON_IRQ_TWSI2.
The interrupts are specified by the device tree and these hard
coded irq numbers block the used of the irq lines by the irq_domain
code.
2) Remove platform device setup code from octeon-platform.c, it is
now unused.
3) Convert i2c-octeon.c to use device tree. Part of this includes
using the devm_* functions instead of the raw counterparts, thus
simplifying error handling. No functionality is changed.
