mark_rodata_ro() and mark_initmem_nx() use functions that can
fail like set_memory_nx() and set_memory_ro(), leading to a not
protected kernel.
In case of failure, panic.

Several places, _PAGE_RW maps to write permission and don't
always imply read. To make it more clear, do as book3s/64 in
commit c7d54842deb1 ("powerpc/mm: Use _PAGE_READ to indicate
Read access") and use _PAGE_WRITE when more relevant.
For the time being _PAGE_WRITE is equivalent to _PAGE_RW but that
will change when _PAGE_READ gets added in following patches.

pte_user() may return 'false' when a user page is PAGE_NONE.
In that case it is still a user page and needs to be handled
as such. So use is_kernel_addr() instead.
And remove "user" text from ptdump as ptdump only dumps
kernel tables.
Note: no change done for book3s/64 which still has it
'priviledge' bit.

update_mmu_cache() voids when hash page tables are not used.
On PPC32 that means when MMU_FTR_HPTE_TABLE is not defined.
On PPC64 that means when RADIX is enabled.
Rename core part of update_mmu_cache() as __update_mmu_cache()
and include the initial verification in an inlined caller.

powerpc has a number of read-only sections and tables that are put after
RO_DATA(). Move the __end_rodata symbol to cover these as well.
Setting memory to read-only at boot is done using __init_begin, change
that to use __end_rodata.
This makes is_kernel_rodata() exactly cover the read-only region, as
well as other things using __end_rodata (e.g., kernel/dma/debug.c).
Boot dmesg also prints the rodata size more accurately.

Currently __init_begin is used as the boundary for strict RWX between
executable/read-only text and data, and non-executable (after boot) code
and data.
But that's a little subtle, so add an explicit symbol to document that
the SRWX boundary lies there, and add a comment making it clear that
__init_begin must also begin there.

Always set an IBAT covering up to _einittext during init because when
CONFIG_MODULES is not selected there is no reason to have an exception
handler for kernel instruction TLB misses.
It implies DBAT and IBAT are now totaly independent, IBATs are set
by setibat() and DBAT by setbat().
This allows to revert commit 9bb162fa26ed ("powerpc/603: Fix
boot failure with DEBUG_PAGEALLOC and KFENCE")

It has been reported some configuration where the kernel doesn't
boot with KASAN enabled.
This is due to wrong BAT allocation for the KASAN area:

A BAT must have both virtual and physical addresses alignment matching
the size of the BAT. This is not the case for BAT 4 above.
Fix kasan_init_region() by using block_size() function that is in
book3s32/mmu.c. To be able to reuse it here, make it non static and
change its name to bat_block_size() in order to avoid name conflict
with block_size() defined in <linux/blkdev.h>
Also reuse find_free_bat() to avoid an error message from setbat()
when no BAT is available.
And allocate memory outside of linear memory mapping to avoid
wasting that precious space.
With this change we get correct alignment for BATs and KASAN shadow
memory is allocated outside the linear memory space.

Some functions defined in 'arch/powerpc/mm' are deserving of an
`__init` macro attribute. These functions are only called by other
initialization functions and therefore should inherit the attribute.
Also, change function declarations in header files to include `__init`.

The two 128K should be a single 256K instead.
When _etext is not aligned to 128Kbytes, the system will allocate
all necessary BATs to the lower 128Kbytes boundary, then allocate
an additional 128Kbytes BAT for the remaining block.
Instead, align the top to 128Kbytes so that the function directly
allocates a 256Kbytes last block:

On book3s/32, the segment below kernel text is used for module
allocation when CONFIG_STRICT_KERNEL_RWX is defined.
In order to benefit from the powerpc specific module_alloc()
function which allocate modules with 32 Mbytes from
end of kernel text, use that segment below PAGE_OFFSET at all time.

Add architecture specific implementation details for KFENCE and enable
KFENCE for the ppc32 architecture. In particular, this implements the
required interface in <asm/kfence.h>.
KFENCE requires that attributes for pages from its memory pool can
individually be set. Therefore, force the Read/Write linear map to be
mapped at page granularity.

Function names should tell what the function does, not how.
mfsrin() and mtsrin() are read/writing segment registers.
They are called that way because they are using mfsrin and mtsrin
instructions, but it doesn't matter for the caller.
In preparation of following patch, change their name to mfsr() and mtsr()
in order to make it obvious they manipulate segment registers without
messing up with how they do it.

early_mmu_init() is independent of MMU type and not
directly linked to tlb handling.
In a following patch, tlb.c will be restricted to HASH mmu.
Move early_mmu_init() into mmu.c which is common.

_tlbie() and _tlbia() are used only on 603 cores while the
other functions are used only on cores having a hash table.
Move them into a new file named nohash_low.S
Add mmu_hash_lock var is used by both, it needs to go
in a common file.

We now have an early hash table on hash MMU, so no need to check
Hash var to know if the Hash table is set of not.
Use mmu_has_feature(MMU_FTR_HPTE_TABLE) instead. This will allow
optimisation via jump_label.

We now always map kernel text with BATs. Neither need to preload
hash with kernel text addresses nor ensure they are never evicted.

Since commit 2b279c0348af ("powerpc/32s: Allow mapping with BATs with
DEBUG_PAGEALLOC"), there is no real situation where mapping without
BATs is required.
In order to simplify memory handling, always map kernel text
and rodata with BATs even when "nobats" kernel parameter is set.
Also fix the 603 TLB miss exceptions that don't require anymore
kernel page table if DEBUG_PAGEALLOC.

At the time being, an early hash table is set up when
CONFIG_KASAN is selected.
There is nothing wrong with setting such an early hash table
all the time, even if it is not used. This is a statically
allocated 256 kB table which lies in the init data section.
This makes the code simpler and may in the future allow to
setup early IO mappings with fixmap instead of hard coding BATs.
Put create_hpte() and flush_hash_pages() in the .ref.text section
in order to avoid warning for the reference to early_hash[]. This
reference is removed by MMU_init_hw_patch() before init memory is
freed.

PowerPC 601 has been retired.
Remove all associated specific code.
CPU_FTRS_PPC601 has CPU_FTR_COHERENT_ICACHE and CPU_FTR_COMMON.
CPU_FTR_COMMON is already present via other CPU_FTRS.
None of the remaining CPU selects CPU_FTR_COHERENT_ICACHE.
So CPU_FTRS_PPC601 can be removed from the possible features,
hence can be removed completely.

In is_module_segment(), when VMALLOC_END is over 0xf0000000,
ALIGN(VMALLOC_END, SZ_256M) has value 0.
In that case, addr >= ALIGN(VMALLOC_END, SZ_256M) is always
true then is_module_segment() always returns false.
Use (ALIGN(VMALLOC_END, SZ_256M) - 1) which will have
value 0xffffffff and will be suitable for the comparison.

The powerpc 32-bit implementation of pgtable has nice shortcuts for
accessing kernel PMD and PTE for a given virtual address. Make these
helpers available for all architectures.

DEBUG_PAGEALLOC only manages RW data.
Text and RO data can still be mapped with BATs.
In order to map with BATs, also enforce data alignment. Set
by default to 256M which is a good compromise for keeping
enough BATs for also KASAN and IMMR.

Mapping RO data as ROX is not an issue since that data
cannot be modified to introduce an exploit.
PPC64 accepts to have RO data mapped ROX, as a trade off
between kernel size and strictness of protection.
On PPC32, kernel size is even more critical as amount of
memory is usually small.
Depending on the number of available IBATs, the last IBATs
might overflow the end of text. Only warn if it crosses
the end of RO data.

At several places pmd pointer is retrieved through the same action:
pmd = pmd_offset(pud_offset(pgd_offset(mm, addr), addr), addr);
or
pmd = pmd_offset(pud_offset(pgd_offset_k(addr), addr), addr);
Refactor this by implementing two helpers pmd_ptr() and pmd_ptr_k()
This will help when adding the p4d level.

hash_page() needs to read page tables from kernel memory. When entire
kernel memory is mapped by BATs, which is normally the case when
CONFIG_STRICT_KERNEL_RWX is not set, it works even if the page hosting
the page table is not referenced in the MMU hash table.
However, if the page where the page table resides is not covered by
a BAT, a DSI fault can be encountered from hash_page(), and it loops
forever. This can happen when CONFIG_STRICT_KERNEL_RWX is selected
and the alignment of the different regions is too small to allow
covering the entire memory with BATs. This also happens when
CONFIG_DEBUG_PAGEALLOC is selected or when booting with 'nobats'
flag.
Also, if the page containing the kernel stack is not present in the
MMU hash table, registers cannot be saved and a recursive DSI fault
is encountered.
To allow hash_page() to properly do its job at all time and load the
MMU hash table whenever needed, it must run with data MMU disabled.
This means it must be called before re-enabling data MMU. To allow
this, registers clobbered by hash_page() and create_hpte() have to
be saved in the thread struct together with SRR0, SSR1, DAR and DSISR.
It is also necessary to ensure that DSI prolog doesn't overwrite
regs saved by prolog of the current running exception. That means:
- DSI can only use SPRN_SPRG_SCRATCH0
- Exceptions must free SPRN_SPRG_SCRATCH0 before writing to the stack.
This also fixes the Oops reported by Erhard when create_hpte() is
called by add_hash_page().
Due to prolog size increase, a few more exceptions had to get split
in two parts.

A few changes to retrieve DAR and DSISR from struct regs
instead of retrieving them directly, as they may have
changed due to a TLB miss.
Also modifies hash_page() and friends to work with virtual
data addresses instead of physical ones. Same on load_up_fpu()
and load_up_altivec().

update_mmu_cache() is only for BOOK3S, and can be simplified for
BOOK3S32.
Move it out of mem.c into respective BOOK3S32 and BOOK3S64 files
containing hash utils.
BOOK3S64 version of hash_preload() is only used locally, declare it
static.
Remove the radix_enabled() stuff in BOOK3S32 version.

When KASAN is selected, the definitive hash table has to be
set up later, but there is already an early temporary one.
When KASAN is not selected, there is no early hash table,
so the setup of the definitive hash table cannot be delayed.

"Slightly delayed due to the issue with printk() calling
probe_kernel_read() interacting with our new user access prevention
stuff, but all fixed now.
The only out-of-area changes are the addition of a cpuhp_state, small
additions to Documentation and MAINTAINERS updates.
Highlights:
- Support for Kernel Userspace Access/Execution Prevention (like
SMAP/SMEP/PAN/PXN) on some 64-bit and 32-bit CPUs. This prevents
the kernel from accidentally accessing userspace outside
copy_to/from_user(), or ever executing userspace.
- KASAN support on 32-bit.
- Rework of where we map the kernel, vmalloc, etc. on 64-bit hash to
use the same address ranges we use with the Radix MMU.
- A rewrite into C of large parts of our idle handling code for
64-bit Book3S (ie. power8 & power9).
- A fast path entry for syscalls on 32-bit CPUs, for a 12-17% speedup
in the null_syscall benchmark.
- On 64-bit bare metal we have support for recovering from errors
with the time base (our clocksource), however if that fails
currently we hang in __delay() and never crash. We now have support
for detecting that case and short circuiting __delay() so we at
least panic() and reboot.
- Add support for optionally enabling the DAWR on Power9, which had
to be disabled by default due to a hardware erratum. This has the
effect of enabling hardware breakpoints for GDB, the downside is a
badly behaved program could crash the machine by pointing the DAWR
at cache inhibited memory. This is opt-in obviously.
- xmon, our crash handler, gets support for a read only mode where
operations that could change memory or otherwise disturb the system
are disabled.
Plus many clean-ups, reworks and minor fixes etc.

Reduce #ifdef mess by defining a helper to print
hash info at startup.
In the meantime, remove the display of hash table address
to reduce leak of non necessary information.

For KASAN, hash table handling will be activated early for
accessing to KASAN shadow areas.
In order to avoid any modification of the hash functions while
they are still used with the early hash table, the code patching
is moved out of MMU_init_hw() and put close to the big-bang switch
to the final hash table.
