Introduce a new mqd allocation interface and split the original
init_mqd function into two functions: allocate_mqd and init_mqd.
Also renamed uninit_mqd to free_mqd. This is preparation work to
fix a circular lock dependency.

Translate queue priority into pipe priority and write to MQDs.
The priority values are used to perform queue and pipe arbitration.

Instead of allocat hiq and sdma mqd from sub-allocator, allocate
them from a mqd trunk pool. This is done for all asics

Wavefront context save data is of interest to userspace clients for
debugging static wavefront state. The MQD contains two parameters
required to parse the control stack and the control stack itself
is kept in the MQD from gfx9 onwards.
Add an ioctl to fetch the context save area and control stack offsets
and to copy the control stack to a userspace address if it is kept in
the MQD.

CU-masking allows a KFD client to control the set of CUs used by a
user mode queue for executing compute dispatches. This can be used
for optimizing the partitioning of the GPU and minimize conflicts
between concurrent tasks.

This commit adds several debugfs entries for kfd:
kfd/hqds: dumps all HQDs on all GPUs for KFD-controlled compute and
SDMA RLC queues
kfd/mqds: dumps all MQDs of all KFD processes on all GPUs
kfd/rls: dumps HWS runlists on all GPUs

The mqd_manager module handles MQD data structures.
MQD stands for Memory Queue Descriptor, which is used by the H/W to
keep the usermode queue state in memory.

Added missing initialization of the addr field in init_mqd()
Setting the hqd persistent.preload_req bit ON so that when queues switches
on/off, their context will kept and read from the mqd when the cp reassign
them, and thus the dispatched workload context kept consistent without any
interrupts.

Change format of mqd structure to match latest KV firmware
