endpoints > 0
The current code assumes that the analog + digital video endpoints are always at
interface number 0 when changing the alternate setting.
This seems to work fine for most existing devices.
However, at least the SpeedLink VAD Laplace webcam has the video endpoint on
interface number 3 (which fortunately doesn't cause any trouble because ist uses
bulk transfers only).
We already consider the actual interface number for audio endpoints, so
rename the the audio_ifnum variable and use it for all device types.
Also get get rid of a pointless (ifnum < 0) in em28xx-audio.

The option to load firmware asynchronously were added due to
a requirement with a few versions of udev. It turns that this was
a bad idea and caused regressions on drxk-based devices.
So, we end by only letting the firmware to be loaded syncronously
everywhere.
So, let's remove the bad code.
This patch partially reverts the changeset 8e30783b0b3.

A few devices (em2860) use a separate interface for audio only
Audio Vendor Class USB. That interface should not be used by
Remote Controller, Analog TV or Digital TV.
Prevents initializing all non-audio extensions for the audio
only interface.

This patch adds support for the UB435-Q V2. You might need to
use the device once with the Windows driver provided by KWorld
in order to permanently reprogram the device descriptors. Thanks
to Jarod Wilson for the initial attempt at adding support for this
device.

Downloading new firmware for DRX-K demodulator is not obligatory but
usually it offers important bug fixes compared to default firmware
burned into chip rom. DRX-K demod driver will continue even without
the firmware, but in that case it will print warning to system log
to tip user he should install firmware.

This is caused by commit c7a45e5b4f8c2f96cd242ae1b1c06e7fb19a08d0,
that added support for two I2C buses. A partial fix was applied
at 3de09fbbfaa521e68675bd30cfece252c4856600, but it doesn't cover
all cases, as the DVB core fills fe->dvb->priv with adapter->priv.

Newer em28xx devices have 2 buses. Change the logic to allow
using both buses.

Putting em28xx in suspend mode when a feed stops is just plain
wrong. Every time a new PES filter is changed, the DVB demux
code will stop the current feed, and then start a new one.
If are there any code that switches off the frontend, via
some GPIO setting, this would make the DVB fail.
This condition was actually trigged with one device, during
DVB scan, as, during scan, it is common that userspace apps
to change the filter several times, in order to get all
tables.
Also, this is not needed at all, since the em28xx code already
hooks into ops.ts_bus_ctrl(). This warrants that em28xx can
check there if DVB frontend is in usage or not. The code there
already puts the device on suspend mode, if the DVB frontend
is not used (closed).

DEV_INITIALIZED of enum em28xx_dev_state state is used nowhere and there is no
need for DEV_MISCONFIGURED, so remove this enum and use a boolean field
'disconnected' in the device struct instead.

The current enpoint logic ignores all bulk endpoints and uses
a fixed mapping between endpint addresses and the supported
data stream types (analog/audio/DVB):
Ep 0x82, isoc => analog
Ep 0x83, isoc => audio
Ep 0x84, isoc => DVB
Now that the code can also do bulk transfers, the endpoint
logic has to be extended to also consider bulk endpoints.
The new logic preserves backwards compatibility and reflects
the endpoint configurations we have seen so far:
Ep 0x82, isoc => analog
Ep 0x82, bulk => analog
Ep 0x83, isoc* => audio
Ep 0x84, isoc => digital
Ep 0x84, bulk => analog or digital**
(*: audio should always be isoc)
(**: analog, if ep 0x82 is isoc, otherwise digital)

The URB data processing for DVB bulk transfers is very similar to
what is done with isoc transfers, so create a common function that
works with both transfer types based on the existing isoc function.
Tested with device Hauppauge HVR-930c.

Rename EM28XX_NUM_PACKETS to EM28XX_NUM_ISOC_PACKETS and
EM28XX_DVB_MAX_PACKETS to EM28XX_DVB_NUM_ISOC_PACKETS to
clarify that these values are used only for isoc usb transfers.
Also use the term num_packets instead of max_packets, as this
is how these values are used and called in struct urb.

This intializes the card just like the windows driver does
- the "HTC USB XS HD" uses the same firmware as the "HTC Stick HD";
- both have different GPIO and reg init sequences;
Tested only with DVB-C.

As em28xx-dvb will always be initialized asynchronously, there's
no need anymore for a separate thread to load the DRX-K firmware.
Fixes a known regression with kernel 3.6 with tda18271 driver
and asynchronous DRX-K firmware load.
Antti tested it with the following hardware:

MaxMedia UB425-TC
PCTV QuatroStick nano (520e)
