The "rate_index" is only used as an index into the phist_data->rx_rate[]
array in the mwifiex_hist_data_set() function. That array has
MWIFIEX_MAX_AC_RX_RATES (74) elements and it's used to generate some
debugfs information. The "rate_index" variable comes from the network
skb->data[] and it is a u8 so it's in the 0-255 range. We need to cap
it to prevent an array overflow.

This patch uses WARN level is not printed by default.
In some cases, some boards have always met the unused log be printed as
follows.

Due to we used the wifi default area was US and didn't support 12~14
channels. As Frequencies:

* 2427 MHz [4] (30.0 dBm)
* 2432 MHz [5] (30.0 dBm)

* 2452 MHz [9] (30.0 dBm)
* 2457 MHz [10] (30.0 dBm)

* 2472 MHz [13] (disabled)
* 2484 MHz [14] (disabled)
Also, as the commit 1b499cb72f26b
("mwifiex: disable channel filtering feature in firmware"), it proved to
be a feature to get better scan result from overlapping channel.
Even there could be AP from overlapping channel (might be 12/13/14
in this case), it will be filtered depend on reg domain rules.
e.g:
...

continue;
So it should not been an ERROR, use the WARN level to instead it for now.

This enum is already perfectly aliased to enum nl80211_band, and
the only reason for it is that we get IEEE80211_NUM_BANDS out of
it. There's no really good reason to not declare the number of
bands in nl80211 though, so do that and remove the cfg80211 one.

It is changed to make it aligned with region code
to country mapping table in driver.

EU is not a valid country in db.txt file. Hence regulatory_hint
returns failure if EEPROM provides region code as 0x30. Let's
use FR for 0x30.

Driver gets country information from EEPROM during
initialization. We will call regulatory_hint to update
current regulatory domain.
As by default world regulatory domain is selected by
cfg80211, country '00' from EEPROM is ignored.
