Several drivers used same function to initialize query MAD,
so move that function to global header file.

The header files in RDMA subsystem are dual licensed and can be
described by simple SPDX tag, so replace all of them at once
together with making them use the same coding style for header
guard defines.

"__attribute__" set of macros has been standardized, have became more
potentially portable and consistent code back in v2.6.21 by commit
82ddcb040 ("[PATCH] extend the set of "__attribute__" shortcut macros").
Moreover, nowadays checkpatch.pl warns about using __attribute__((packed))
instead of __packed.
This patch converts all the "__attribute__ ((packed))" annotations to
"__packed" within the RDMA subsystem.

When the hfi1 driver was added these definitions were moved from the qib driver
to ib_mad.h to be used by both qib and hfi1. They should have been moved to
ib_smi.h instead.

The base versions handle constant folding just fine, use them
directly. The replacements are OK in the include/ files as they are
not exported to userspace so we don't need the __ prefixed versions.
This patch does not affect code generation at all.

Move ipath's struct port_info into <rdma/ib_smi.h>, so that it can be
used by mthca to implement client reregister support.
Remove the __attribute__((packed)) because all the members of the struct
are naturally aligned anyway.

Move the InfiniBand headers from drivers/infiniband/include to include/rdma.
This allows InfiniBand-using code to live elsewhere, and lets us remove the
ugly EXTRA_CFLAGS include path from the InfiniBand Makefiles.
