SIGTRAP handler
Clear the software event flag in the augmented SS to prevent immediate
repeat of single step trap on return from SIGTRAP handler if the trap
flag (TF) is set without an external debugger attached.
Following is a typical single-stepping flow for a user process:
1) The user process is prepared for single-stepping by setting
RFLAGS.TF = 1.
2) When any instruction in user space completes, a #DB is triggered.
3) The kernel handles the #DB and returns to user space, invoking the
SIGTRAP handler with RFLAGS.TF = 0.
4) After the SIGTRAP handler finishes, the user process performs a
sigreturn syscall, restoring the original state, including
RFLAGS.TF = 1.
5) Goto step 2.
According to the FRED specification:
A) Bit 17 in the augmented SS is designated as the software event
flag, which is set to 1 for FRED event delivery of SYSCALL,
SYSENTER, or INT n.
B) If bit 17 of the augmented SS is 1 and ERETU would result in
RFLAGS.TF = 1, a single-step trap will be pending upon completion
of ERETU.
In step 4) above, the software event flag is set upon the sigreturn
syscall, and its corresponding ERETU would restore RFLAGS.TF = 1.
This combination causes a pending single-step trap upon completion of
ERETU. Therefore, another #DB is triggered before any user space
instruction is executed, which leads to an infinite loop in which the
SIGTRAP handler keeps being invoked on the same user space IP.

The first GDT descriptor is reserved as 'NULL descriptor'. As bits 0
and 1 of a segment selector, i.e., the RPL bits, are NOT used to index
GDT, selector values 0~3 all point to the NULL descriptor, thus values
0, 1, 2 and 3 are all valid NULL selector values.
When a NULL selector value is to be loaded into a segment register,
reload_segments() sets its RPL bits. Later IRET zeros ES, FS, GS, and
DS segment registers if any of them is found to have any nonzero NULL
selector value. The two operations offset each other to actually effect
a nop.
Besides, zeroing of RPL in NULL selector values is an information leak
in pre-FRED systems as userspace can spot any interrupt/exception by
loading a nonzero NULL selector, and waiting for it to become zero.
But there is nothing software can do to prevent it before FRED.
ERETU, the only legit instruction to return to userspace from kernel
under FRED, by design does NOT zero any segment register to avoid this
problem behavior.
As such, leave NULL selector values 0~3 unchanged and close the leak.
Do the same on 32-bit kernel as well.

A control-protection fault is triggered when a control-flow transfer
attempt violates Shadow Stack or Indirect Branch Tracking constraints.
For example, the return address for a RET instruction differs from the copy
on the shadow stack.
There already exists a control-protection fault handler for handling kernel
IBT faults. Refactor this fault handler into separate user and kernel
handlers, like the page fault handler. Add a control-protection handler
for usermode. To avoid ifdeffery, put them both in a new file cet.c, which
is compiled in the case of either of the two CET features supported in the
kernel: kernel IBT or user mode shadow stack. Move some static inline
functions from traps.c into a header so they can be used in cet.c.
Opportunistically fix a comment in the kernel IBT part of the fault
handler that is on the end of the line instead of preceding it.
Keep the same behavior for the kernel side of the fault handler, except for
converting a BUG to a WARN in the case of a #CP happening when the feature
is missing. This unifies the behavior with the new shadow stack code, and
also prevents the kernel from crashing under this situation which is
potentially recoverable.
The control-protection fault handler works in a similar way as the general
protection fault handler. It provides the si_code SEGV_CPERR to the signal
handler.

There are significant differences between signal handling on 32-bit vs.
64-bit, like different structure layouts and legacy syscalls. Instead
of duplicating that code for native and compat, merge both versions
into one file.

When setup frame fails, force_sigsegv is called and returns -EFAULT.
There is similar code in ia32_setup_frame(), ia32_setup_rt_frame(),
__setup_frame() and __setup_rt_frame().
Make them identical.
No change in functionality intended.

On cpu's supporting xsave/xrstor, fpstate pointer in the sigcontext, will
include the extended state information along with fpstate information. Presence
of extended state information is indicated by the presence
of FP_XSTATE_MAGIC1 at fpstate.sw_reserved.magic1 and FP_XSTATE_MAGIC2
at fpstate + (fpstate.sw_reserved.extended_size - FP_XSTATE_MAGIC2_SIZE).
Extended feature bit mask that is saved in the memory layout is represented
by the fpstate.sw_reserved.xstate_bv
For RT signal frames, UC_FP_XSTATE in the uc_flags also indicate the
presence of extended state information in the sigcontext's fpstate
pointer.

move 64bit routines that saves/restores fpstate in/from user stack from
signal_64.c to xsave.c
restore_i387_xstate() now handles the condition when user passes
NULL fpstate.
Other misc changes for prepartion of xsave/xrstor sigcontext support.

dynamically allocate fpstate on the stack, instead of static allocation
in the current sigframe layout on the user stack. This will allow the
fpstate structure to grow in the future, which includes extended state
information supporting xsave/xrstor.
signal handlers will be able to access the fpstate pointer from the
sigcontext structure asusual, with no change. For the non RT sigframe's
(which are supported only for 32bit apps), current static fpstate layout
in the sigframe will be unused(so that we don't change the extramask[]
offset in the sigframe and thus prevent breaking app's which modify
extramask[]).

This makes the x86 signal handling code use tracehook_signal_handler() in
place of calling into ptrace guts. The call is moved after the sa_mask
processing, but there is no other change. This cleanup doesn't matter to
existing debuggers, but is the sensible thing: have all facets of the
handler setup complete before the debugger inspects the task again.

Right, I never liked having to fiddle with those TIF flags. Initially I
needed it because the hrtimer base lock could not nest in the rq lock.
That however is fixed these days.
Currently the only reason left to fiddle with the TIF flags is remote
wakeups. We cannot program a remote cpu's hrtimer. I've been thinking
about using the new and improved IPI function call stuff to implement
hrtimer_start_on().
However that does require that smp_call_function_single(.wait=0) works
from interrupt context - /me looks at the latest series from Jens - Yes
that does seem to be supported, good.
Here's a stab at cleaning this stuff up ...
