Function debugfs_remove() recursively removes a directory, include all
files created by debugfs_create_file(). Therefore, there is no need to
explicitly record each file with member ->bnad_dentry_files[] and
explicitly delete them at the end. Remove field bnad_dentry_files[] and
its related processing codes for simplification.

Driver bna can work fine even if any previous call to debugfs create
APIs failed. All return value checks of them should be dropped, as
debugfs APIs say.

Currently, we allocate a nbytes-sized kernel buffer and copy nbytes from
userspace to that buffer. Later, we use sscanf on this buffer but we don't
ensure that the string is terminated inside the buffer, this can lead to
OOB read when using sscanf. Fix this issue by using memdup_user_nul
instead of memdup_user.

We could allocate less memory than intended because we do:
bnad->regdata = kzalloc(len << 2, GFP_KERNEL);
The shift can overflow leading to a crash. This is debugfs code so the
impact is very small.

sscanf(kern_buf, "%x:%x", &addr, &val);
and used as arguments for bna_reg_offset_check and writel
so they have to be unsigned.

...and remove some of them. It is not necessary to log when .probe() and
.remove() are called or when TxQ is started or stopped. Also log level
of some of them was changed to more appropriate one (link up/down,
firmware loading failure.

Patch converts kzalloc->copy_from_user sequence to memdup_user. There
is also one useless assignment of NULL to bnad->regdata as it is followed
by assignment of kzalloc output.

alloc failures use dump_stack so emitting an additional
out-of-memory message is an unnecessary duplication.

Change details:
- Add debugfs support to obtain firmware trace, saved firmware trace on
an IOC crash, driver info and read/write to registers.
- debugfs hierarchy:
bna/pci_dev:<pci_name>
where the pci_name corresponds to the one under /sys/bus/pci/drivers/bna
- Following are the new debugfs entries added:
fwtrc: collect current firmware trace.
fwsave: collect last saved fw trace as a result of firmware crash.
regwr: write one word to chip register
regrd: read one or more words from chip register.
drvinfo: collect the driver information.
