The thuge-gen test program runs mmap() and shmget() tests for both every
available page size and the default page size, resulting in two tests for
the default size. These tests are distinct since the flags in the default
case do not specify an explicit size, add the flags to the test name that
is logged to deduplicate.

The thuge-gen test_mmap() and test_shmget() tests are repeatedly run for a
variety of sizes but always report the result of their test with the same
name, meaning that automated sysetms running the tests are unable to
distinguish between the various tests. Add the supplied sizes to the
logged test names to distinguish between runs.
My test automation was getting pretty confused about what was going on
- the test names are a pretty important external interface.

Centralize the _GNU_SOURCE definition to CFLAGS in lib.mk. Remove
redundant defines from Makefiles that import lib.mk. Convert any usage of
"#define _GNU_SOURCE 1" to "#define _GNU_SOURCE".
This uses the form "-D_GNU_SOURCE=", which is equivalent to
"#define _GNU_SOURCE".
Otherwise using "-D_GNU_SOURCE" is equivalent to "-D_GNU_SOURCE=1" and
"#define _GNU_SOURCE 1", which is less commonly seen in source code and
would require many changes in selftests to avoid redefinition warnings.

thuge-gen.c defines SHM_HUGE_* macros that are provided by the uapi since
4.14. These macros get redefined when compiling with Android's bionic
because its sys/shm.h will import the uapi definitions.
However if linux/shm.h is included, with glibc, sys/shm.h will clash on
some struct definitions:

Conform the layout, informational and status messages to TAP. No
functional change is intended other than the layout of output messages.
Also remove unneeded logging which isn't enabled. Skip a hugepage size if
it has less free pages to avoid unnecessary failures. For examples, some
systems may not have 1GB hugepage free. So skip 1GB for testing in this
test instead of failing the entire test.

commit 23baf831a32c ("mm, treewide: redefine MAX_ORDER sanely") has
changed the definition of MAX_ORDER to be inclusive. This has caused
issues with code that was not yet upstream and depended on the previous
definition.
To draw attention to the altered meaning of the define, rename MAX_ORDER
to MAX_PAGE_ORDER.

thuge-gen was previously only munmapping part of the mmapped buffer, which
caused us to run out of 1G huge pages for a later part of the test. Fix
this by munmapping the whole buffer. Based on the code, it looks like a
typo rather than an intention to keep some of the buffer mapped.
thuge-gen was also calling mmap with SHM_HUGETLB flag (bit 11 set), which
is actually MAP_DENYWRITE in mmap context. The man page says this flag is
ignored in modern kernels. I'm pretty sure from the context that the
author intended to pass the MAP_HUGETLB flag so I've fixed that up too.
