Nothing calls snd_msndmidi_new()
thus nothing sets chip->msndmidi_mpu
The call to snd_msndmidi_input_read is gated on that being set,
so snd_msndmidi_input_read() won't be called either.
This is probably a missing call to snd_msndmidi_new(), but since
this is ancient code, it's probably best to remove it (especially
since I don't have the hardware to test it).

The io-mapped buffers used in msnd drivers need __iomem annotations.
This fixes sparse warnings like:

snd_msnd_pcm() takes a pointer to a pointer of a PCM where if this parameter
is provided the newly allocated PCM is stored. All callers pass NULL though,
so remove the parameter. This makes the code a bit cleaner and shorter.

CONFIG_HOTPLUG is going away as an option. As result the __dev*
markings will be going away.
Remove use of __devinit, __devexit_p, __devinitdata, __devinitconst,
and __devexit.

This is driver for Turtle Beach Multisound cards:
Classic, Fiji and Pinnacle.
Tested pcm playback and recording and MIDI playback
on Multisound Pinnacle.
