Remove the check for IOMMU presence since it was considered a
layer violation.
This means we have no reliable way to destinguish between coherent
hardware IOMMU DMA address translations and incoherent SWIOTLB DMA
address translations, which we can't handle. So always presume the
former. This means that if anybody forces SWIOTLB without also setting
the vmw_force_coherent=1 vmwgfx option, driver operation will fail,
like it will on most other graphics drivers.

When calling vmw_fb_set_par(), the mode stored in par->set_mode gets free'd
twice. The first free is in vmw_fb_kms_detach(), the second is near the
end of vmw_fb_set_par() under the name of 'old_mode'. The mode-setting code
only works correctly if the mode doesn't actually change. Removing
'old_mode' in favor of using par->set_mode directly fixes the problem.

If it's not a system error and get_node implementation accommodate the
buffer object then it should return 0 with memm::mm_node set to NULL.

Commit 2db76d7c3c6d ("lib/scatterlist: sg_page_iter: support sg lists w/o
backing pages") introduced the sg_page_iter_dma_address() function without
providing a way to use it in the general case. If the sg_dma_len() is not
equal to the sg length callers cannot safely use the
for_each_sg_page/sg_page_iter_dma_address combination.
Resolve this API mistake by providing a DMA specific iterator,
for_each_sg_dma_page(), that uses the right length so
sg_page_iter_dma_address() works as expected with all sglists.
A new iterator type is introduced to provide compile-time safety against
wrongly mixing accessors and iterators.

During modeset check it is possible to have all crtc_state's in atomic
state. Check for crtc enable status while checking for display unit
active status. Only error if enabling a crtc while display unit is not
active.

The function was unconditionally returning 0, and a caller would have to
rely on the returned fence pointer being NULL to detect errors. However,
the function vmw_execbuf_copy_fence_user() would expect a non-zero error
code in that case and would BUG otherwise.
So make sure we return a proper non-zero error code if the fence pointer
returned is NULL.

This is only used by drm_irq_install(), which is an optional helper.
For legacy pci devices this is required (due to interrupt sharing without
msi/msi-x), and just making this the default exactly matches the behaviour
of all existing drivers using the drm_irq_install() helpers. In case that
ever becomes wrong drivers can roll their own irq handling, as many
drivers already do (for other reasons like needing a threaded interrupt
handler, or having an entire pile of different interrupt sources).

If a non-legacy driver calls these it's valid to assume there is
interrupt support. The flag is really only needed for legacy drivers,
which control IRQ enabling/disabling through the DRM_IOCTL_CONTROL
legacy IOCTL.
Also remove all the flag usage from non-legacy drivers.

Since Linux 4.21 we merged the swiotlb ops into the DMA direct ops,
so they would always have a the sync_single methods. But late in
the cicle we also removed the direct ops entirely, so we'd see NULL
DMA ops. Switch vmw_dma_select_mode to only detect swiotlb presence
using swiotlb_nr_tbl() instead.

The function ttm_bo_put releases a reference to a TTM buffer object. The
function's name is more aligned to the Linux kernel convention of naming
ref-counting function _get and _put.
A call to ttm_bo_unref takes the address of the TTM BO object's pointer and
clears the pointer's value to NULL. This is not necessary in most cases and
sometimes even worked around by the calling code. A call to ttm_bo_put only
releases the reference without clearing the pointer.
In places where is might be necessary, the current behaviour of cleaning the
pointer is kept.

Having the probe helper stuff (which pretty much everyone needs) in
the drm_crtc_helper.h file (which atomic drivers should never need) is
confusing. Split them out.
To make sure I actually achieved the goal here I went through all
drivers. And indeed, all atomic drivers are now free of
drm_crtc_helper.h includes.
v2: Make it compile. There was so much compile fail on arm drivers
that I figured I'll better not include any of the acks on v1.
v3: Massive rebase because i915 has lost a lot of drmP.h includes, but
not all: Through drm_crtc_helper.h > drm_modeset_helper.h -> drmP.h
there was still one, which this patch largely removes. Which means
rolling out lots more includes all over.
This will also conflict with ongoing drmP.h cleanup by others I
expect.
v3: Rebase on top of atomic bochs.
v4: Review from Laurent for bridge/rcar/omap/shmob/core bits:
- (re)move some of the added includes, use the better include files in
other places (all suggested from Laurent adopted unchanged).

- provide a common DMA_MAPPING_ERROR definition and avoid indirect
calls for dma_map_* error checking

retpoline overhead for high performance workloads

- provide a generic remapping DMA consistent allocator for
architectures that have devices that perform DMA that is not cache
coherent. Based on the existing arm64 implementation and also used

- improve the dma-debug infrastructure, including dynamic allocation

Avoid expensive indirect calls in the fast path DMA mapping
operations by directly calling the dma_direct_* ops if we are using
the directly mapped DMA operations.

With the new validation code, a malicious user-space app could
potentially submit command streams with enough buffer-object and resource
references in them to have the resulting allocated validion nodes and
relocations make the kernel run out of GFP_KERNEL memory.
Protect from this by having the validation code reserve TTM graphics
memory when allocating.

The return statement is redundant as there is a return statement
immediately before it so we have dead code that can be removed.
Also remove the unused declaration of ret.

This fixes a layout update race condition. We make sure
the crtc mutex is locked before we dereference crtc->state. Otherwise the
state might change under us.
Since now we're already holding the crtc mutexes when reading the gui
coordinates, protect them with the crtc mutexes rather than with the
requested_layout mutex.

Make the connector is_implicit property immutable.
As far as we know, no user-space application is writing to it.
Also move the verification that all implicit display units scan out
from the same framebuffer to atomic_check().
