Going forward, struct intel_display is the main display device data
pointer. Convert intel_modeset_setup.[ch] to struct intel_display.

Split out the modeset hardware state readout and sanitize, or state
setup, to a separate file.
Do some drive-by checkpatch fixes while at it.
